摘  要:目的:探讨兔严重胸腹伤后早期莫沙必利促肠道动力治疗对胃肠道缺血性损害的影响。方法:采用BIM-IV型生物撞击机建立实验性胸腹撞击伤动物模型,将30只新西兰兔分为正常对照组、创伤组和创伤促动力组,观察门静脉血流量、外周静脉和门静脉乳酸水平及胃和小肠黏膜形态学变化。结果:创伤促动力组门静脉血流量下降程度明显较小(P<0.05),外周静脉和门静脉血乳酸浓度升高程度明显低于创伤组(P<0.01),并且胃黏膜损害较轻。结论:兔严重胸腹伤后早期采用莫沙必利促胃肠道动力治疗对改善胃肠道缺血性损害有一定的保护作用。Objective To investigate the efficiency of a prokinetic agent,mosapride,on gastrointestinal tract ischemia in early stage of severe thoraco-abdominal injury. Methods A total of 30 New Zealand white rabbits were randomly divided into normal control group (n=6),trauma group (n=12) and mosapride treatment group (n=12). The experimental model of thoraco-abdominal injury was established with BIM-IV biological impact machine. Mosapride (10 mg/kg) was injected intraduodinally at 0.5 and 6.5 h after injury. Portal venous flow,lactic acid concentration in the portal vein and peripheral vein,morphological change of stomach and small intestine mucosa were measured and observed at 2 and 8 h after injury. Results Compared with trauma group,mosapride treatment group had obviously decreased portal venous flow (P0.05),lower lactic acid concentration in the portal vein and peripheral vein (P0.01) though it was increased than normal control,and lighter pathological impairment of stomach mucosa. Conclusion Mosapride exerts an early promoting gastrointestinal motility effect to protect gastrointestinal tract from ischemia injury after severe thoraco-abdominal injury.
