冠心病患者外周血单个核细胞粒钙蛋白基因表达的变化

摘　　要：目的:探讨外周血单个核细胞粒钙蛋白(GCA)基因表达水平与冠心病的关系。方法:连续入选接受冠状动脉造影(CAG)检查的稳定型心绞痛(SAP)患者,对照组为接受CAG检查排除冠心病者。CAG前完成空腹静脉取血进行血常规和超敏C-反应蛋白(hs-CRP)检测、提取总核糖核酸(RNA)并合成互补脱氧核糖核酸(cDNA)进行实时定量RT-PCR(quantitative real-time reveres transcriptase-polyme-rase chain reaction)。结果:SAP组(n=61)糖尿病比例高于对照组(n=53)(34.4%vs15.1%,P<0.05),淋巴细胞分类计数低于对照组(27.2%vs31.7%,P<0.05)。SAP组GCA基因表达水平高于对照组[(1.09±0.41)vs(0.86±0.51),P<0.01]。GCA基因表达水平与hs-CRP以及白细胞分类计数水平均不存在相关性(均P>0.05)。另将样本分为糖尿病组(n=29)和无糖尿病组(n=85)进行比较,2组间GCA基因表达水平差异无统计学意义[(0.97±0.44)vs(0.98±0.48),P>0.05]。结论:稳定型心绞痛患者的外周血单个核细胞GCA基因表达水平升高,提示其持续高表达可能与冠心病有关。Objective：To evaluate the expression level of grancalcin（GCA） in peripheral blood mononuclear cells（PBMC）.Methods：Patients were classified into two groups as documented by coronary angiography.Sixty-one patients with coronary artery disease who had stable effort angina pectoris（SAP） and had ≥ 50% diameter stenosis in left main coronary artery or ≥75% diameter stenosis in at least one major coronary artery.Controls were those who with chest pain,abnormal ecletronic cadiogram or abnormal echocardiography,but had a smooth coronary angiogram（CAG）.Venous blood was obtained before CAG for RNA isolation and routine blood count.The total RNA were used for individual cDNA preparation.Expression levels of genes in individuals were evaluated by quantitative real time RT-PCR.Results：The prevalence of diabetes was higher in the patients with SAP than the controls（34.4% vs 15.1%,P = 0.032）,and the frequency of combining arteriosus sclerosis obliterans（ASO） and/or ischemic stroke history was higher in the former than that in the latter（19.7% vs 0%,P = 0.0006）.WBC count and subset counts were not different between SAP and control groups（all P 0.05）,except that relative lymphocyte count in percentage was lower in the former than that in the later（27.2% vs 31.7%,P = 0.02）.Expression level of GCA was higher in SAP than that in the control [（1.09 ± 0.41） vs（0.86 ± 0.51）,P =0.008].There were no correlation between GCA and WBC subset counts or hs-CRP level（all P 0.05）.Expression levels of GCA did not differ between diabetic（n = 29） andnon-diabetic（n = 85） patient groups[（0.97 ± 0.44） vs（0.98 ± 0.48）,P 0.05].Conclusion：Up-regulation of grancalcin gene expression in peripheral blood mononuclear cells may be associated with coronary artery disease.
