音乐疗法对初次行永久性起搏器植入术患者情绪及生理状况的影响

摘　　要：目的探讨音乐疗法对初次行永久性起搏器植入术患者情绪及生理状况的影响作用。方法将113例初次行永久起搏器植入术患者随机分为两组,对照组(56例)采用常规护理,音乐组(57例)在常规护理基础上实施音乐疗法,两组患者分别于入院时及术前夜、手术后8h采用Zung焦虑自评量表(self rating anxiety scale,SAS)测评患者焦虑情况;两组均采用McGill疼痛强度指数评估患者术后伤口、腰背疼痛情况,并记录术后入睡困难及排尿困难情况;两组患者均于入院时、手术前夜、手术日清晨、入手术室前测量心率、血压,并作比较。结果两组患者术前夜、术后8hSAS评分比较,均P<0.01,差异具有统计学意义,音乐组得分低于对照组;两组患者术后8h腰背疼痛、入睡困难和排尿困难等不适感比较,均P<0.05,差异具有统计学意义,音乐组发生率明显低于对照组;音乐组干预后3次收缩压、心率测量值均比对照组低(均P<0.05)。结论音乐疗法可有效降低初次行永久性起搏器植入术患者的焦虑程度,明显提高患者的舒适度,并有稳定患者生命体征的作用。Objective To assess the efficacy of music therapy in patients with initial permanent pacemaker implantation.Methods 113 patients who received a permanent pacemaker implantation initially were randomly separated into two groups：56 patients in conventional care group and 57 patients in music therapy group on the basis of conventional care.Adopting international certification Zung self-rating auxiety Scale（SAS）,we measured patients＇ anxiety the day they were admitted into hospital,the night right before operation and 8 hours after operation respectively.We evaluated postoperative pain degree at the wound and the back by McGill pain intensity index（PPI） and postoperative sleep and urination.At the same time,we compared the patients＇ heart rate and blood pressure at 7：00 am after admission,the night right before operation,the morning on the day of operation and the moment before entering into intervention room.Results In comparison with the single conventional care,the SAS scores in the night right before operation and 8 hours after operation in the musicotherapy group were significantly lower than those in the control group（P 0.01）;the postoperative discomforts like pain and difficulty in falling asleep and urination,were weaker（P 0.05）.The patients＇ systolic blood pressures and heart rates after musicotherapy were improved（P 0.05）.Conclusion Music therapy can effectively relieve anxiety of the permanent pacemaker implantation patients,ease their discomfort,and stabilize their vital signs.
