新生儿经鼻持续正压通气简易鼻塞固定连接管路的研制及应用

摘　　要：目的:探讨自制简易鼻塞固定连接管路对新生儿经鼻持续正压通气治疗的临床疗效。方法:117例患儿随机分为对照组(40例)和治疗组(77例),治疗组应用自制简易鼻塞连接管路和机器连接对新生儿经鼻行持续正压通气治疗;对照组应用机器自带的鼻塞固定管路。观察临床有效率、动态动脉血气分析监测结果、经皮氧饱和度上升正常时间、鼻黏膜和面部发生损伤的情况、鼻塞自行脱落的次数。结果:(1)治疗组有效73例,有效率为94.8%,4例无效(5.2%);对照组有效36例,有效率为90.0%,无效4例(10.0%),经χ2检验,差异有统计学意义(P<0.01)。(2)治疗组患儿治疗前,治疗后12h、24h、48h的pH、SaO2、PaO2、PaCO2,与对照组比较无差异(P>0.05);治疗后4h治疗组的pH、SaO2、PaO2、PaCO2改善情况明显优于对照组,差异有统计学意义(P<0.01)。(3)治疗组患儿每人每天鼻塞脱离的次数为(3±1.2),而对照组患儿每人每天鼻塞脱离的次数为(5±0.9),经t检验分析,t=13.54,差异有统计学意义(P<0.01)。(4)治疗组的患儿不良反应的发生率为6.8%,对照组为22.2%,经χ2检验,差异有统计学意义(P<0.01)。结论:该简易鼻塞固定连接装置制造成本低,临床效果好,并发症的发生率低,安全可靠,可以代替其他的同类产品应用,值得进一步推广应用。Objective：To observe the clinical efficacy of simple stuffed-up nose fixedly conjunction tube for neonatal nasal continuous positive airway pressure ventilation.Methods：One hundred and seventeen patients who had neonatal respiratory distress syndrome were randomly divided into treatment group（n=77）and control group（n=40）.Treatment group accepted the treatment of using simple stuffed-up nose fixedly conjunction tube for neonatal nasal continuous positive airway pressure ventilation.Control groups accepted the treatment of using traditional nose fixedly conjunction tube.The effective rate,dynamic blood gas analysis,the ascension normal time of the skin oxygen saturation degree,harm of the nasal circumstance,the number of times for the stuffed-up nose shed off by oneself were observed.Results：The efficacy of treatment group（94.8%） was better than that of control group（90.0%,P0.01）.The level of pH,SaO2,PaO2,PaCO2 for treatment group were no difference than that of control group before treatment and after treatment for 4hs,12hs,24hs,48hs（P0.05）.The level of pH、SaO2、 PaO2、PaCO2 for treatment group were most significantly advanced than that of control group after the treatment for 4hs（P0.01）.The number of times for the stuffed-up nose shed off by oneself for treatment group（3±1.2）was better than that of control group（5±0.9）（t=13.54,P0.01）.The incidence rate of complication for treatment group（6.8%）was most lower than that of control group（22.2%,P0.01）.Conclusion：The cost of simple stuffed-up nose fixedly conjunction tube was low and the clinical efficacy was effective.The incidence rate of complications was low.It can replace other kind of the same products and worth expanding and applicating further.
