人格诊断问卷在心理治疗病房患者中的使用结果

摘　　要：目的用人格诊断问卷(PDQ+4)调查心理治疗病房患者的人格异常情况,初步了解心理治疗病房人格障碍倾向的分布情况。方法以PDQ+4问卷为工具,调查心理治疗病房年满18岁的住院患者,获有效问卷76份。结果不同性别、年龄组和婚姻状况的住院患者分别在PDQ+4若干分量表上有差异(P<0.05),其中30岁及以上组的反社会型(1.00±1.00)、回避型(3.87±1.89)和抑郁型分量表(3.67±2.13)上均为最低(P<0.05)。人格障碍阳性率为强迫型(92.1%)和回避型(77.6%)最高,分裂性(31.6%)最低。患者PDQ+4总分为(52.68±15.87),与国内其他研究类似。样本PDQ+4总阳性率达到676.4%,其筛查结果和临床诊断并不一致。因子分析提取了3个因子,与DSM-IV诊断标准并不相符。结论心理治疗病房的患者较其他人群总体上有明显的人格异常,值得关注。PDQ+4作为自评问卷有一定价值,但在临床使用时,需要其他工具配合。Objectives To investigate inpatients of ward for psychotherapy with PDQ＋4,to study the distribution of the inpatients＇ personality disorder. Methods Investigate inpatients who are at least 18 years old with PDQ＋4,76 qualifi ed results are got. Results Inpatients with different gender,age,marital status have different results in PDQ＋4＇s subscale（P〈0.05） . Inpatients over 30 years old get the lowest score in antisocial personality disorder（1.00±1.00） ,avoidant personality disorder（3.87±1.89） and depressive personality disorder（3.67±2.13）（P〈0.05） . Obsessive and compulsive personality disorder（92.1%） and avoidant personality disorder（77.6%） have the highest positive rate. Schizoid personality disorder（31.6%） has the lowest positive rate. Inpatients’ scores are similar with the scores of other researchers. The total positive rate is 676.4%,different to the clinical diagnose. 3 factors of PDQ＋4 are distilled by Factor analysis,but they do not correspond with DSM-IV diagnosis criterion. Conclusion Inpatients of psychotherapy ward have obvious personality abnormality compared with other patients. PDQ＋4 is a useful tool as a self-report scale,but it should be corporate with other tool in clinical situation.
