摘 要:目的通过检测骨骼肌中泛素和20S蛋白酶体亚单位HC3 mRNA的表达,探讨腹部创伤/感染患者骨骼肌蛋白分解的非溶酶体降解机制,为改进腹部创伤/感染患者营养支持治疗提供依据。方法选择腹部创伤/感染患者30例作为A组,另选腹部良性病变择期手术患者30例作为B组。抽取外周静脉血,采用ELISA法检测2组患者血清白细胞介素-6(IL-6)。术中切取腹壁横纹肌组织,用RT-PCR方法检测泛素、HC3 mRNA表达情况。结果 A组横纹肌泛素、HC3 mRNA表达水平高于B组,静脉血IL-6水平也高于对照组。横纹肌泛素、HC3 mRNA表达水平与静脉血IL-6水平成正比,与患者营养状态成反比。结论泛素-蛋白酶体降解途径在腹部创伤/感染患者骨骼肌耗损及蛋白质降解中起着重要作用。针对泛素-蛋白酶体途径改进营养支持治疗,有助于改善腹部创伤/感染患者营养状况。Objective It is to detect the ubiquitin and 20S proteasome subunit HC3 mRNA expression in skeletal muscle to study the protein breakdown and non-lysosomal degradation mechanism of skeletal muscle in abdominal trauma/infection pa- tients, so to provide evidences for the improvement of these patients' nutritional support treatment. Methods 30 cases of pa- tients with abdominal trauma/ infection were chosen as group A, and 30 cases of patients with abdominal benign lesions going to accept elective abdominal surgery were chosen as control group. Peripheral venous blood of the patients in both groups was collected to determine serum IL -6 by ELISA assay. Ubiquitin and HC3 mRNA expression in abdominal wall striated muscle cut during surgery was detected by RT - PCR method. Results In comparison with control group B, the expression levels of striated muscle ubiquitin and HC3 mRNA were higher in group B, the level of blood IL - 6 was also higher in group B. Striat- ed muscle ubiquitin, HC3 mRNA expression levels were positively proportional to IL -6 level, and inversely proportional to nutritional status of patients. Conclusion The ubiquitin-proteasome pathway plays an important role in depletion of skeletal muscle and protein degradation with the abdominal trauma/infection patients. Adjustment of nutritional support therapy according to the ubiquitin-proteasome pathway, might improve the nutritional status of abdominal trauma/infection patients.
