罗哌卡因腰硬联合麻醉用于老年患者前列腺手术临床观察

摘　　要：目的探讨不同浓度等剂量轻比重罗哌卡因腰硬联合麻醉用于老年患者前列腺电汽化手术的效果。方法选择65-91岁前列腺增生老年患者90例,随机分成三组（每组30例）。Ⅰ、Ⅱ、Ⅲ组依次应用0.25%、0.5%、0.75%轻比重罗哌卡因10 mg行腰硬联合麻醉。观察其感觉、运动阻滞上的起效、维持及恢复的异同,最高阻滞平面,术中麻醉质量及出现的不良反应与并发症。结果三组患者患侧感觉阻滞起效时间与最高阻滞平面比较差异无统计学意义;运动阻滞起效时间、感觉和运动阻滞维持时间、最大B rom age评分,硬膜外追加局麻药比例比较差异有统计学意义。结论 0.5%罗哌卡因腰硬联合麻醉用于老年患者前列腺电汽化手术时最具有优越性。Objective To study the effect of ropivacaine at different concentrations and equal close light proportion on spinal and epidural anesthesia for prostate electrovaporization of elderly patients.Methods 90 cases of elderly patients（65 to 91 years old） with prostate hyperplasia were divide into 3 groups.The patients of Ⅰ,Ⅱ,and Ⅲ groups were carried out spinal and epidural anesthesia using 10 mg with light proportion ropivacaine at concentration of 0.25%,0.5%,and 0.75%.The effect and maintain retrieve with sense,movement,the tiptop block plane,anesthesia quality in operation,the badness feedback and syndrome were observed.Results There was no significant difference in effect of feeling block in trouble side and tiptop block plane among 3 groups.There was statistics significance in time of movement block effect,feeling movement block maintenance,and most Bromage grade.Conclusion The spinal and epidural anesthesia with the 0.5% ropivacaine had great advantage in prostate electrovaporization for elderly patients.
