综合干预预防阴囊筋膜网蒂皮瓣重建尿道术后并发症的效果观察

摘　　要：目的探讨综合干预方法预防阴囊筋膜网蒂皮瓣重建尿道术后并发症的效果。方法将110例行阴囊筋膜网蒂皮瓣重建尿道术患者应用随机数字表法分为对照组51例,干预组59例。对照组常规采用0.1%聚维酮碘(0.1%PVP-1)清洗尿道口;生理盐水10～15ml冲洗新尿道支架管。干预组采用综合干预方法:生理盐水l0～15ml加氯霉素眼药水5ml冲洗新尿道支架管;0.1%聚维酮碘(0.1%PVP-1)清洗尿道口;红外线照射伤口局部;用脂质水胶体敷料优拓包扎术后伤口。观察两组术后并发症的发生情况。结果对照组发生局部切口感染6例、局部组织血液循环不良9例、局部组织血肿6例、皮瓣坏死2例、尿液引流不畅9例、尿瘘6例;干预组发生局部切口感染1例、局部组织血液循环不良2例、局部组织血肿1例、尿液引流不畅3例、尿瘘1例:两组比较除皮瓣坏死外,差异均有统计学意义(P>0.05)。结论阴囊筋膜网蒂皮瓣重建尿道术后实施综合干预,能降低尿道下裂术后并发症发生率,提高手术成功率,促进患者早日康复。Objective To explore the effect of comprehensive nursing interventions in reducing postoperative complications in patients with scrotal faseial flap reconstruction of urethra. Methods 110 patients with scrotal fascial flap reconstruction of urethra were randomly divided into intervention group and control group. In control group, urethral meatus was washed with O.I%PVP-1 and new urethral stent was flushed with 10-15 ml normal saline. In intervention group, following nursing interventions were applied. （1） Flushing the new urethral stent with 10-15 ml normal saline and 5 ml chloramphenicol eye drops; （2） washing urethra with O.I%PVP- 1; （3） applying infrared radiation in local wound; （4） covering the wound with urgoful （a lipid hydroeolloid dressing） at last. Results There were statistical significances of incidences of complications between the two groups, such as urinary fistula, would infection, hematoma, while flap necrosis had not. Conclusion Comprehensive nursing interventions could reduce postoperative complications in patients with scrotal fascial flap reconstruction of urethra.
