输尿管支架管移位的原因分析及对策

摘　　要：目的探讨输尿管支架管(双J管)移位的原因、处理防范及预防措施。方法对16例双J管移位患者的临床资料进行回顾分析。结果 3例下移的双J管取出后重新放置,2例开放手术取出上移的双J管,6例用输尿管镜取出上移的双J管,5例上移者用预留的丝线拖出双J管。结论术前应测量肾盂至膀胱的长度选择合适的双J管;放置双J管时应先充分放置膀胱端,尾端预留丝线是取管的简易方法。术后第1周观察双J管是否移位相当重要。Objective To investigate the cause,prevention and management of double-J stent displacement.Methods Clinical data of 16 cases with migrated double-J stent was retrospectively analyzed.Results Three cases with descending double-J were replaced,2 cases with the ascending double-J were moved out by open surgery,6 cases were removed by ureteroscopy,and 5 cases by the ＂pull string＂ supplied with the stent.Conclusions The length of the ureter should be measured in order to choose a suitable double J stent.The end of the stent must be placed into the bladder,and reserving a ＂pull-string＂ suture is a simple and easy method for removing the stent.It is necessary to observe the location of the double-J stent during the first week after operation.
