摘  要:目的分析腰围(WC)和血压(BP)对代谢综合征(MS)的预测作用。方法采取横断面研究方法对4 134名长沙市某单位员工进行问卷调查,检测身高、体重、WC、BP、FPG、血脂等。用非条件Logistic回归法分析WC和BP对诊断MS贡献的大小。结果 (1)MS的粗患病率为24.4%(男25.8%,女20.6%),按人口普查结果标准化后患病率为17.3%(男19.2%,女15.8%)。(2)用WC、BMI、SBP、DBP预测该人群MS的一致性为90.2%;若去掉BMI,预测一致性为89.3%。在其他因素不变时,WC每增加5cm、SBP每增加10mmHg、DBP每增加5mmHg,可分别增加MS的患病风险10.84倍、1.20倍及1.27倍。结论联合测量WC和BP对MS有较强的预测作用,可作为一项快速诊断和评估MS的简易指标。Objective To clarify the association of waist circrnference and blood pressure with metabolic syndrome(MS). Methods A cross-sectional study was used and 4134 subjects aged 19-80 years (male 3029, female 1105)were enrolled. The all subjects were interviewed by trained health providers under a structured questionnaire. The height, body weight, waist circumference (WC) and blood pressure were examined in these subjects. Meanwhile, fasting plasma glucose(FPG), serum total cholesterol(TC), triglyceried(TG), high density lipoprotein cholesterol(HDL-C) were also tested. Multiple logistic stepwise regression analysis was used to identify the association of waist circumference and blood pressure with MS. Results The crude prevalences of MS were 24. 4% in total population (25. 8% in male,20. 6% in female) and age-standardized rates were 17. 3% (19.2% in male,15. 8~ in female). The combination of WC,BMI,SBP and DBP accounted for 90. 2% of the variance of MS,and if BMI was deleted,the rest three items accounted for 89. 3% of the variance. An elevating of 5cm in WC,or 10mmHg in SBP,or 5mmHg in DBP would increase the risk for MS by 10. 84 times, 1.20 times and 1.27 times, respectively. Conclusions The combination of WC and blood pressure can be used as a simple predictor for MS in medical clinical practice.
