摘 要:目的研究不同糖耐量患者血清Vaspin水平变化与胰岛素敏感性等相关因素的关系。方法 89例研究对象分为正常对照、糖耐量减低和2型糖尿病3组。检测人体测量参数和生化指标,留取血样测FPG、FIns、血脂、HbA_1c、Vaspin水平,采用稳态模式评估胰岛素抵抗指数(HOMA-IR)等,分析这些参数在各组人群间的关系。结果 (1)2型糖尿病组女性血清Vaspin水平显著高于男性(1.22±0.35 vs1.01±0.27,P<0.05);(2)三组人群血清Vaspin水平无明显差异(1.13±0.47 vs 1.09±0.33 vs 1.11±0.33,P>0.05);(3)在对性别、年龄、身高、体重、BMI进行校正后,血清Vaspin与FIns成正相关(r=0.244,P<0.05)与LDL-C呈负相关(r=-0.266,P<0.05)。FIns、HOMA-IR、BMI是影响血清Vaspin的独立相关因素。结论空腹血清Vaspin水平正相关于空腹胰岛素,负相关于LDL-C;FIns,HOMA-IR和BMI是影响血清Vaspin的独立相关因素。Objective To investigate the correlation of serum Vaspin with insulin sensitivity and its related factors in different glucose tolerance individuals. Methods 89 subjects were divided into three groups:normal glucose tolerance (NGT, n = 32), impaired glucose tolerance (IGT, n = 20)and type 2 diabetes mellitus(T2DM, n= 37). After measurement of the height, body weight and waist circumference, the fasting blood samples were collected from all participants to determine levels of fasting plasma glucose (FPG), serum insulin, blood lipids, HbA^c, and Vaspin. Homeostasis model assessment of insulin resistance(HOMA-IR) was calculated based on fasting PG and insulin levels. The relationship between these parameters in each group was analyzed. Results (1) Vaspin level was higher in female than in male (1. 224-0. 35 vs 1. 014-0. 27,P〈0. 05)only in T2DM,not in IGT and NGT group. (2)Serum Vaspin levels were not significantly different among T2DM, IGT and NGT groups (all P〉 0. 05) (3) Serum Vaspin levels were correlated positively with Fins and negatively with LDL-C(r= 0. 244, r= - 0. 266, P〈0. 05 ) after adjusting for age, sex, height, body weight and BMI. Multiple regression analysis showed that Fins, HOMA-IR and BMI were independently related factors influencing serum Vaspin levels. Conclusions Fasting serum Vaspin level is associated with insulin resistance and obesity in patients with abnormal glucose level.
