急性白血病多药耐药基因表达的临床研究

摘  要:通过综合研究不同病期急性白血病、慢粒急变及骨髓增生异常综合征原始细胞过多性难治性贫血(MDSRAEB)患者mdrl表达,研究发现其与预后的关系,以指导临床诊断及治疗。采用RTPCR技术,并设置β2微球蛋白(β2M)作内标研究各组病例mdrl基因表达情况。急性白血病、慢粒急变及MDSRAEB患者mdrl基因表达阳性率均明显增高,急性白血病难治/复发组mdrl基因阳性表达率明显高于急性白血病初治组与完全缓解组;初治白血病治疗前mdrl阴性者,经正规化疗后,CR率明显高于mdrl阳性组。mdrl基因表达增加是白血病细胞固有的特性,mdrl表达增加能导致细胞耐药。The mdrl expression of acute leukemia, blast crisis of CML and MDS RAEB patients in peripheral blood were studied to find their relationship with prognosis and to guide clinical diagnosis and treatment. PCR technique was used with β 2M as the intrinsic index to study the mdrl gene expression. The positive mdrl gene expression of acute leukemia, blast crisis of CML and MDS RAEB patients were obviously elevated. The positive rate of mdrl gene expression of radical/relapsed acute leukemia were higher than that of untreated and CR acute leukemia. Mdrl negative untreated leukemia patients had higher CR rate than mdrl positive ones after chemotherapy. The increasing mdrl gene expression is the intrinsic characteristics of leukemic cells and the increasing mdrl expression can lead to drug resistance of cell,lower the effect of chemotherapy.
