154例慢性肺心病急性发作期中医证候的回顾性调查

摘　　要：目的:对慢性肺心病急发期中医证候分布规律进行研究,为中医诊治本病提供规范化研究依据。方法:对154例肺心病急性发作期病例进行回顾性调查。根据患者的一般资料、中医证候学及实验室检查资料,分非老年组和老年组2组,探索本病中医证候分布规律。结果:本病急发期中医证型以热痰内蕴型与热痰夹瘀型为最多见,约占50%～60%,其他证型占40%～50%。非老年组与老年组中气虚血瘀型分别为8.0%、11.5%,气阴两虚型分别为10.0%、4.8%,阳虚水泛型分别为10.8%、14.4%,痰浊闭窍型分别为10.0%、9.6%,老年组中寒痰内蕴型占3.8%。结论:热痰证、血瘀证、痰浊证等是肺心病急发期"标"实的主要表现,气虚证或气阴两虚证或阳虚证等是本病"本"虚的主要表现。Objective：To investigate the distribution pattern of TCM syndrome in patients with acute exacerbation of chronic pulmonary heart disease to provide standardized basis for diagnosis and treatment.Methods：a retrospective survey was adopted to collect the materials of 154 patients.including general information,TCM syndrome and laboratory findings,for exploring the distribution pattern of TCM syndrome of patients.They were separated into two groups of non-elderly and elderly group.Results：The TCM syndromes as phlegm-heat in the lung and heat-sputum with blood stasis were the most frequently encountered,with the proportion of 50% ～ 60%,the other syndromes were found in the proportion of 40% ～ 50%.the qi deficiency and blood stasis syndrome was the probability of 8.0% and 11.5% respectively.The syndromes as qi and yin deficiency were in 10.0% and 4.8% respectively.The syndrome of Yang and water pan was in 10.8% and 14.4% respectively.The syndrome of turbid phlegm blocking the mind was in 10.0% and 9.6% respectively,the lowest probability（3.8%）in the syndrome of cold sputum in the lung.Conclusion：The heat-sputum and blood stasis syndrome and turbid phlegm syndrome etc.are the principal clinical manifestation of excess in superficiality.The qi deficiency or both qi and yin deficiency or yang deficiency syndrome etc.is the main clinical manifestation of the deficiency in origin.
