婴幼儿血液中肌酐正常参考值的研究

摘　　要：目的为婴幼儿血液中肌酐含量提供一个更为合理的正常参考范围,以指导临床在诊断、观察、治疗和判断婴幼儿疾病时,能更准确地分析和了解病情的变化。方法对我院儿保门诊和部分非肾病科的住院患者,年龄从出生至16岁的1 419例婴幼儿作血清肌酐测定,按年龄分组与成年组的237例进行统计学分析。结果从得到的结果经统计分析显示,5岁以下婴幼儿组的参考范围应为22.75～63.55μmol/L(平均为43.15μmol/L),占成年组平均含量的50.43%。5～16岁儿童组的参考范围应为43.36～92.96μmol/L(平均为68.16μmol/L),占成年组平均含量的79.65%。结论健康婴幼儿血清中肌酐浓度与成年组血清中的肌酐浓度有着显著性差异,其中5岁以下的幼儿组的差别最大,其平均含量仅为成年组平均含量的50.43%。儿童组的平均含量与成年组的差别略有减少,为成年组平均含量的79.65%。而5岁以下的幼儿组与5岁～16岁的儿童组有显著性差异(P<0.01)。说明婴幼儿的血液中的肌酐含量有随年龄的增长而增加。各年龄组肌酐在性别上无明显差异。Objective We establish a more reasonable reference intervals of serum creatinine in infants and young children to guide the clinical diagnosis,observation and treatment more accurately.Methods 1 419 cases of children,were enrolled from outpatients of children health care center and in-patients without nephrotic disease,aged from birth to 16 years old.Statistically analysis was proceeded and compared with the average of 237 adults.Results The reference intervals of serum creatinine under 5 years old were 22.75～63.55 μmol/L（mean level 43.15 μmol/L）,which account for 50.43% of the adult.As to 5～16 years old,the reference interval was 43.36～92.96 μmol/L（mean level 68.16 μmol/L）,which account for 79.65% of the adult.Conclusion The serum creatinine concentration had a significant difference between infants and adults,the reference interval under 5 years old was 22.75-63.5 umoul/L,as to the 5～16 years old,the reference interval was 43.36-92.96 umol/L,.and infants under 5 years old had a significant difference with children aged from 5 to 16years old,it also showed serum creatinine levels of infants was increasing with age,but.significant difference was not found in gender .
