设置特需护理员对降低早发型母乳性黄疸的作用

摘　　要：目的探讨设置特需护理员对降低早发型母乳性黄疸发生率的效果。方法 2009年1月～12月在孕妇入院时,将双日入院者安排到母婴同室一区单间,为实验组,共268对母子;将单日入院者安排到母婴同室二区单间,为对照组,共178对母子。实验组设置特需护理员配合责任护士,负责母乳喂养知识和技巧的具体指导,对照组不配置特需护理员。比较2组新生儿母乳喂养频率、第一次排胎便时间、经皮胆红素值及早发型母乳性黄疸的发生率。结果实验组新生儿生后第1天母乳喂养频率大于对照组新生儿(p<0.01)、第一次排胎便时间早于对照组(p<0.01);生后第2天、第3天,实验组新生儿经皮胆红素值及早发型母乳性黄疸的发生率均低于对照组新生儿(p均<0.01),差别有统计学意义。结论在母婴同室设立特需护理员协助责任护士,负责对产妇进行母乳喂养的具体指导和健康教育,可降低早发型母乳性黄疸的发生率。Objective：To figure out the effect of special nursing assistant settings on breast feeding jaundice.Methods：During January to December in 2009,268 postnatal women who were admitted to hospital on even days were assigned in the first ward of room of mother and infant,which was called intervention group.The 178 postnatal women who were admitted to hospital on odd days were assigned in the second ward of room of mother and infant,which was called control group. In the intervention group,sepeeial nursing assistant helped the nurses to direct breast feeding,while in the control group,there was not special nursing assistant setting.The frequenee of breast feeding,the time of first defecation,the transcutaneous bilirubin of the infants,and the incidence of breast feeding jaundice were compared with the two groups.Results：The frequenee of breast feeding on Day I in intervention group was higher than that of the control group （p〈0.01）,the time of first defecation was earlier than that of the control group （p〈0.01）.On Day 2 and Day 3,both the incidence of the transeutaneous bilirubin of the infants and the incidence of breast feeding jaundice in intervention group were lower than that of the control group（p〈0.01,p〈0.01）.Conclusion：The special nursing assistants in rooms of mother and infant who help the nurses direct breast feeding and health education could help decrease the incidence of breast feeding jaundice.
