氟比洛芬酯用于泌尿外科腔镜手术术后镇痛的效果观察

摘　　要：目的:观察氟比洛芬酯用于泌尿外科腔镜手术术后镇痛的有效性和安全性,比较氟比洛芬酯超前镇痛和氟比洛芬酯常规镇痛的效果差异。方法:选择ASA Ⅰ-Ⅱ级择期行泌尿外科腔镜手术的患者120例,随机分为氟比洛芬酯超前镇痛组(A组)、氟比洛芬酯常规镇痛组(B组)、安慰剂组(C组),均实施腰-硬联合麻醉。A组术前15min静推氟比洛芬酯注射液1mg/kg,B组手术结束前15min静推氟比洛芬酯注射液1mg/kg,C组术前15min静推10ml生理盐水。记录患者术后3、6、12、24h视觉模拟(VAS)评分、镇痛满意度、追加补救镇痛药的例数和时间及不良反应的发生例数。结果:A组术后6、12、24hVAS评分明显低于同时间点的C组,B组术后6h、12hVAS评分明显低于同时间点的C组(P<0.05)。镇痛满意度评价A、B两组均明显优于C组(P<0.05)。补救镇痛药物的使用例数A、B两组均明显少于C组(P<0.05),使用时间A组长于B、C两组、B组长于C组。三组不良反应的发生情况无差别。结论氟比洛芬酯用于泌尿外科腔镜手术术后镇痛是安全有效的,并且氟比洛芬酯超前镇痛的效果要优于常规镇痛。Objective： To observe the efficacy and safety of flurbiprofen ester for urological surgery analgesia, to compare the defferent effects between flurbiprofen pre-emptive analgesia ester and conventional analgesia ester. Method ：To select 120 patients with urological surgery in ASA i - ii class with elective suegery, and divide them into flurbiprofen ester group of pre-emptive analgesia （grouo A）,the flurbiprofen ester conventional analgesia group（group B）and the placebo group （group C） randomly,with implementing spinal-epidural anesthesia to all of them.Group A should be given 15-minute preoperative intravenous flurbiprofen injection of lmg/kg,group B should be given injection of intravenous flurbiprofen of 1 mg/kg 15 minutes before the end of the surgery,group C should be given static push of lOml saline before the he surgery.To recored the patients VAS,the satisfation of pain,the case numbers and time of the additional analgesic remedies and the number of cases of adverse reactions in 3h,6h,12h, 24h after tile surgery.Result：The VAS of group A in 6h,12h,24h after the surgery is significantly lower than group B at the same time, the VAS of group B in 6h,12h,24h after the surgery is significantly lower than group C at the same time.The evaluation of satisfaction with analgesia of group A and group B is much better than group C. The case numbers and time of the additional analgesic remedies of group A and group B is significantly less than group C,the use of the time of group A is much longer than group B and group C with group B is longer than group C.The incidence of adverse reactions of them has no defference.Conclusion： It＇s safe and effective of flurbiprofen use to the anagcsia after laparoscopic surgery for urological with the effect of preemptive analgesia is superior to conventional analgesic.
