摘 要:目的评价超声和X线钼靶检查及两者联合应用对小乳腺癌的诊断价值。方法回顾性分析116个乳腺小肿块(恶性59个、良性57个)的术前超声与钼靶直接和间接征象及两者联合应用结果,将病变分级为良性、可能良性、不能确定、可能恶性及恶性,分别与手术病理结果对照分析,应用受试者工作特征曲线(ROC曲线)评价三者的诊断效果。结果超声和钼靶联合诊断乳腺小肿块ROC曲线下面积(0.938±0.025)大于单独超声(0.901±0.031)及钼靶(0.895±0.031),且有统计学意义(P均<0.05)。结论超声与X线铜靶检查各有优缺点,且具有互补性,直接和间接征象对判断肿块性质至关重要,两者联合应用能提高小乳腺癌的诊断准确性。Objective To evaluate the examination of ultrasonography, mammography and their combination in diagnosis of small breast cancers. Methods The sonographic and mammographic direct or indirect signs of 116 small breast lesions (59 malignant, 57 benign) were retrospectively analyzed. The lesions were defined as five categories of benign, probably benign, equivocal, probably malignant and malignant. The results of ultrasonography, mammography and their combination were compared with pathology. By using the definitive diagnosis and the five levels of suspicion categories, receiver operating characteristic (ROC) curves were drawn to evaluate their diagnostic results. Results The area under the ROC curve of combination (0. 938±0. 025) was larger than that of ultrasonography (0. 901±0. 031) and mammography (0. 895±0. 031) . The increase in the area was significant (P〈0.05). Conclusion Both uhrasonography and mammography have their own superiority and shortcoming. Ultrasonography and mammography can be a supplement to each other. Direct and indirect signs of small breast lesions are very important for diagnosis. Combination of uhrasonography and mammography can result in an increase in diagnostic accuracy of small breast cancers.
