摘　　要：目的静脉窦型房间隔缺损,传统的经皮房间隔缺损封堵术因腔静脉处无房间隔残缘,为该术的禁忌证,既往均需在体外循环下作房间隔缺损修补术,本研究应用经食管超声心动图(TEE)指导、监测在微创、无需作体外循环的条件下行静脉窦型房间隔缺损杂交术(Hybrid),避免了体外循环和创伤较大的修补术。方法 11例静脉窦型房间隔缺损患者,术前将探头插入食管内,测量缺损大小、部位和周边的解剖关系,提供术者选择封堵器大小、置放和缝合部位,整个Hybrid手术过程在TEE指导、监测下完成。结果 11例静脉窦型房间隔缺损患者均获得成功,术后即刻TEE检查均未测及残余分流。结论静脉窦型房间隔缺损行Hybrid术,在TEE指导和监测下,能准确测量缺损大小,了解部位及与周边解剖关系,供术者选择封堵器大小、置放和缝合部位参考。Objective To evaluate the value of transesophageal echocardiography （TEE） in Hybrid Surgery of transthoracic minimally invasive occlusion of sinus venosus atrial septaI defect （ASD） . Methods Eleven patients with sinus venosus ASD were enrolled. The size, positon and anatomic edge of ASD were estimated by TEE in all the patients for selecting optimal occlusion device. The procedures of Hybrid surgery were guided and monitored by TEE, too. Results All the procedures were successful. No residual shunt was detected out instantaneously after the operation by TEE. Conclusions TEE plays an important role in Hybrid surgery of transthoracic minimally invasiveocclusion of sinus venosus ASD. It could offer accurate size, positon and anatomic edge of ASD for selecting optimal occlusion device and navigating Hybrid surgery.
