摘 要:目的比较代谢综合征(metabolic syndrome,MS)患者和非MS的健康对照者唾液中肿瘤坏死因子α(TNF—α)、白细胞介素(IL)-1β水平,了解牙周炎与MS患者体内低度炎症的关系。方法采用病例对照的研究方法,共纳入MS患者57例(MS组),非MS的全身健康者26人(健康组)。留取受检者静息全唾液,用放射免疫的方法检测TNF-α的质量浓度,酶联免疫吸附试验检测IL-1β的质量浓度。比较MS组和健康组唾液中细胞因子水平的差异,并分析细胞因子与牙周临床指标和MS组分数的相关关系。结果MS组唾液中TNF—α水平[(69.30±21.01)ng/L]显著高于健康组[(57.85±15.69)ng/L],P〈0.05;唾液中IL-1β的水平[(616.42±360.05)ng/L]高于健康组[(506.06±245.76)ng/L],差异无统计学意义(P〉0.05)。唾液中的TNF-α水平与探诊后出血呈正相关(r=0.220,P〈0.05)。唾液中TNF-α和IL-1β水平在MS组中随发生MS的组分数增加而增高、随牙周炎程度加重而增高,但差异均无统计学意义。结论牙周感染可能是MS患者体内低度炎症的来源之一,唾液TNF-α、IL-1β水平可能不同程度地受全身状况和口腔感染的影响。Objective To investigate the association between periodontitis and the low-grade inflammation in metabolic syndrome(MS) patients. Methods Fifty-seven MS patients, 26 healthy controls were enrolled. Non-stimulated whole saliva was collected. The levels of tumor necrosis factor (TNF)-α and interleukin (IL)-1βwas analyzed by radioimmunoassay and enzyme-linked immunosorbeut assay, respectively. Concentration of cytokines was compared between MS patients and the healthy controls. Correlations between the eytokines and various periodontal indices, and between the cytokines level and different quantity of metabolic syndrome components were also investigated. Results Levels of TNF-α in saliva of MS patients[ (69. 30±21.01 ) ng/L] were significantly higher than that in the healthy subjects L (57.85 ±15.69) ng/L,P 〈0. 05], and of IL-β1 in MS patients[ (616.42 ±360. 05) ng/L] higher than that in healthy subjects (506.06±245.76 ) ng/L] , but the difference was not statistically significant. TNF-α was positively correlated with bleeding index (BI). In MS patients, TNF-α level and IL-1β level increased with increasing severity of periodontal disease and increasing component numbers of MS. Conclusions Periodontal inflammation may be one of the sources of low-grade inflammation in MS patients. Both systemic and periodontal conditions may influence the level of salivary TNF-α and IL-1β
