乳腺癌组织及血浆中RASSF1A,CDH1甲基化的检测及临床意义

摘　　要：目的 研究乳腺癌组织、乳腺良性肿瘤组织及相应患者血浆中Ras相关区域家族1A（RASSF1A）、上皮型钙黏附素（CDH1）基因启动子区异常甲基化状态及其临床意义.方法 应用SYBR GreenⅠ实时荧光定量PCR方法对乳腺癌组织、乳腺良性肿瘤组织及相应血浆中RASSF1A,CDH1基因甲基化状态进行检测.结果 34例乳腺癌组织RASSF1A基因启动子甲基化率为73.53%（25/34）,CDH1基因启动子甲基化率为50.00%（17/34）,相应血浆中RASSF1A的甲基化检出率为55.88%（19/34）;CDH1的甲基化检出率为35.29%（12/34）;联合检测血浆中RASSF1A和CDH1基因启动子区甲基化的敏感性为64.71%（22/34）;而25例良性肿瘤组织RASSF1A基因启动子甲基化率为12.00%（3/25）,CDH1基因启动子甲基化率为4.00%（1/25）,相应血浆中未检测出甲基化的RASSF1A和CDH1基因.RASSF1A,CDH1甲基化率与肿瘤病理类型、患者年龄、有无淋巴结转移的差异无显著性（P〉0.05）.结论 联合检测血浆中RASSF1A和CDH1基因启动子区甲基化,可用于乳腺癌的辅助诊断.Objective To investigate the clinical significance of aberrant methylation in the promoter area of RASSFI A and CDH 1 in human breast carcinoma tissue and serum. Methods The aberrant methylation in the promoter area of RASSF1A and CDH 1 in breast tissue and serum of patients with breast carcinoma and with benign breast tumor were detected by SYBR Green I real-time quantitative PCR （ RT-PCR ）. Results The detection rate of methylation of RASSF1A in breast cancer tissues was 73.53% （ 25/34 } and in serum was 55. 88% （ 19/34 ）. The detection rate of methylation of CDH1 in breast cancer tissues was 50.00% （ 17/34 ） and in serum was 35.29% （ 12/ 34）. The combined detection rate of methylation of RASSF1A and CDH 1 in serum was 64.71% （22/34） in 34 patients with breast cancer. The detection rate of methylation of RASSF1 A and CDH1 in benign breast tumor was 12.00% （ 3/25 ） and 4.00% （ 1/25 ） respectively. The methylation of RASSF1A or CDH 1 in serum was not detected in 25 patients with benign breast tumor. There was no significant correlation between the detection rate of methylation of RASSF1A or CDH1 with pathologic types, the age, and lymphnode metastasis（ P 〉 0.05 ）. Conclusion Combining detection of methylation of RASSF1 A and CDH 1 in serum may be used as auxiliary diagnosis for breast cancer.
