摘 要:目的 探讨盐酸戊乙奎醚对脑肿瘤病人脑氧代谢及s100b蛋白的影响.方法 选择ASA错误ASAⅠ或Ⅱ及择期手术病人60例,随机分为3组:盐酸戊乙奎醚0.02mg/kg(A组),盐酸戊乙奎醚0.05mg/kg(B组)和安慰剂对照组(C组).3组均采用复合全身麻醉,诱导前5min各组分别给予相应剂量的盐酸戊乙奎醚或安慰剂.分别于诱导前即刻(T1)、诱导后30min(T2)和手术结束前(T3)取动脉血和颈内静脉球血行血气分析,计算动脉血氧含量(CaO2),颈内静脉血氧含量(CjvO2),动静脉血氧含量差[C(a-jv) O2],脑氧摄取率(CERO2),动静脉血乳酸浓度差VADL;并测定T1,T2和T3时点颈内静脉血血清s100B蛋白和NSE含量.结果 3组病人各时点CaO2无统计学差异.A,B组CjvO2,C(a-jv) O2,VADL在T2,T3时刻与T1比较有统计学意义(P〈0.05),且与同时刻C组比较均有显著差异(P〈0.05).A,B组CERO2在T2,T3时刻与T1比较有统计学意义(P〈0.05).3组在T2,T3时刻血清S100B蛋白和NSE含量较T1均明显升高(P〈0.05),且C组升高最为显著(P〈0.01);在T2,T3时刻A,B组与同时刻C组比较,血清S100B蛋白和NSE含量变化均有统计学意义(P〈0.05).结论 盐酸戊乙奎醚可明显改善脑组织缺氧,提高氧摄取率,维持脑氧供需平衡,减轻术后脑损伤程度,具有脑保护作用.Objective To investigate the effect of Penehyclidine Hydrochloride on cerebral o2 supply/consumption and protein s100b in patients with cerebral tumor. Methods Sixty ASA I or II patients undergoing elective surgery were randomly divided into 3 groups. The patients in the three groups were given combined anesthesia. Five minutes before anesthesia induction, group A was injected Pene- hyelidine Hydrochloride(0.02mg/kg) ,group B was injected Penehyclidine Hydrochloride (0.05mg/kg) ,while group C was injected placebo. Blood samples were taken for blood gas analysis from artery( a) and jugular venous bulb(jv) simultaneously before anesthesia induction (T1 ) ,30min after anesthesia induction(T2) and before conclusion of operation. Arterial oxygen content,arterial-jugular bulb venous O2 content difference ( Da-jvO2 ) , cerebral O2 extraction rate ( CERO2 ) and arterial-jugular bulb venous lactate difference (VADL) were calculated. At the same time, protein s100b and NSE were detected. Results In group A and B, the level of CjvO2 on T2 and T3 was higher than that of on T1 ( P 〈 0.05 ). Compared with group C, CERO2 , Protein s100b and NSE of three groups on T2 and T3 were higher than that of on T1 ( P 〈 0.05 ) , and group C had the highest level( P 〈 0.01 ). Compared with group C ,the level of Protein s100b and NSE of group A and B was lower on T2 and T3 ( P 〈 0.05 ). Conclusion Penehyclidine Hydrochloride can increase cerebral O2 supply and extraction rate. Penehyclidine Hydrochloride has effect on cerebral protection.
