补肾固精方合大剂量黄芪注射液对脾肾阳虚型狼疮肾炎免疫状态的影响

摘　　要：目的观察补肾固精方结合大剂量黄芪注射液对脾肾阳虚型狼疮肾炎免疫状态的影响。方法将30例脾肾阳虚型狼疮肾炎患者随机分为治疗组和对照组各15例,两组均采用强的松加环磷酰胺(CTX)冲击治疗,治疗组同时予补肾固精方和黄芪注射液;治疗3个月后观察两组24h尿蛋白定量、SLE疾病活动指数(SLEDAI)、T细胞亚群、调节性T细胞(CD4+CD25+Treg细胞)、IL-17变化情况和治疗中感染事件发生情况。结果治疗组可明显降低脾肾阳虚型狼疮肾炎患者24h尿蛋白定量、SLEDAI和感染事件发生率,下调CD8+水平,且疗效优于对照组(P<0.05);同时能提高CD4+CD25+Treg细胞比率,降低IL-17水平,但与对照组比较无统计学差异(P>0.05)。结论补肾固精方结合大剂量黄芪注射液可明显改善脾肾阳虚型狼疮肾炎病情,并可在一定程度上纠正免疫功能紊乱。Objective To observe the clinical effects of ＂Bushen Gujing Decoction＂ plus high-dose Astragalus injection on immunological condition in patients with lupus nephritis（LN） of spleen-kidney yang-deficiency.Methods Thirty LN patients were randomly divided into treatment group（n=15） and control group（n=15）;steroids and cyclophosphamide（CTX） were administered to all patients,while ＂Bushen Gujing Decoction＂ plus Astragalus injection were only administered to patients in the treatment group.After 3-month treatment,24h urine protein,SLE disease activity index score（SLEDAI score）,T lymphocytes subsets,CD4＋CD25＋ regulatory T cell（CD4＋CD25＋Tr cell）,plasma IL-17 levels,and infection incidence were detected in both groups.Results Compaired with control group,treatment group could significantly decrease 24h urine protein,SLEDAI score,infection rate and down-regulate the level of CD8＋T cell（P0.05）;but there was no significant difference in up-regulating CD4＋CD25＋Treg cell and decreasing plasma IL-17 in both groups（P0.05）.Conclusion ＂Bushen Gujing Decoction＂ plus high-dose Astragalus injection can significantly ameliorate LN of spleen-kidney yang-deficiency by correcting immune dysfunction.
