摘  要:目的探讨吡非尼酮(pirfenidone,PDF)对单侧输尿管梗阻(UUO)大鼠肾间质纤维化的作用。方法雌性SD大鼠42只,随机分为3组:假手术组、模型组及治疗组[PDF500mg/(kg.d)]。UUO术后7d和14d分别处死各组大鼠,光镜观察梗阻肾组织病理变化;免疫组化和RT-PCR方法检测各组肾组织α-平滑肌肌动蛋白(α-SMA)的表达水平;RT-PCR方法检测各组肾组织结缔组织生长因子(CTGF)的表达水平。结果模型组肾脏间质纤维化病变最明显,治疗组病变显著减轻(P<0.01)。与假手术组相比,模型组α-SMA蛋白和基因表达及CTGF的基因表达均明显增加(均P<0.01);治疗组上述物质表达则较模型组明显减少(均P<0.01)。结论吡非尼酮可能通过抑制α-SMA和CT-GF的表达进而减少细胞外基质的沉积,发挥肾脏保护作用。Objective To investigate the effect of pirfenidone on renal interstitial fibrosis after unilateral urethral obstruction(UUO)in rats.Methods Forty-two female Sprague-Dawley rats were randomly divided into three groups:control group(C,undergoing sham operation),UUO group(M),and UUO group treated with pirfenidone(S,500 mg/kg every day).Rats were sacrificed at day 7 and 14 after UUO operation,and the renal tissues were examined by PAS and Masson stain.Immunohistochemistry and reverse transcription-polymerase chain reaction were applied to detect the protein and mRNA expression of α-SMA,and reverse transcription-polymerase chain reaction was applied to determine mRNA expression of CTGF.Results Tubulointerstitial fibrosis was found in group M.As compared with group M,the lesion was remarkably attenuated in group S(P〈0.01).As compared with group C,the protein and mRNA expression of α-SMA and CTGF was increased significantly in group M(P〈0.01).As compared with group M,the expression of α-SMA and CTGF in group S was decreased significantly(P〈0.01).Conclusion Pirfenidone may protect the kidney by inhibiting the expression of α-SMA and CTGF and subsequently reducing the extracellular matrix deposition.
