前路减压内固定治疗胸腰椎爆裂骨折并截瘫的研究

摘　　要：目的探讨前路减压内固定治疗胸腰椎爆裂骨折并截瘫的临床疗效。方法回顾评价63例胸腰椎爆裂骨折并截瘫病例经前路减压内固定手术治疗的临床疗效。结果所有病例脊髓均获得有效减压,45例获得随访,术后平均随访4.8年,发现植骨块融合良好,伤椎高度基本恢复,Cobb's角由术前平均17°恢复到5°,Frankel分级恢复一级者16例,恢复二级者15例,无变化者14例。结论前路减压内固定术是集减压、复位、内固定、植骨融合、矫正畸形、重建脊柱稳定一次完成的有效方法,但应严格掌握手术指征。Objective To investigate the anterior decompression and internal fixation of thoracolumbar burst fracture and paraplegia of the clinical efficacy. Methods A retrospective evaluation of 63 cases of thora- eolumbar burst fracture and paraplegia cases were anterior decompression and internal fixation in all eases the results of the clinical efficeacy of spinal cord are an effective decompression, 45 eases were followed after an av- erage follow-up of 4. 8 years and found that bone block integration of good, basic resume injured vertebral height, Cobb＇ s angle from preoperative average of 17 o to return to 5 o, Frankel grade in 16 patients to resume a restore the two in 15 cases, no change in 14 cases. Conclusion Anterior decompression and internal fixation is a decompression, reduction, internal fixation, bone fusion and correction of deformity, reconstruction of spi- nal stability in an effective way to complete, but should be stricdy controlled surgical indications.
