氧化低密度脂蛋白抗体对动脉粥样硬化诊断的临床价值研究

摘　　要：目的探讨与基质金属蛋白酶-9比较,血清中氧化低密度脂蛋白抗体在诊断动脉粥样硬化中的临床价值.方法采用酶联免疫吸附法(ELISA)和RT-PCR技术分别检测30例动脉粥样硬化患者、67例冠心病患者(稳定型心绞痛组36例和急性心肌梗死组31例)及34例健康者血清氧化低密度脂蛋白抗体水平和全血中基质金属蛋白酶-9的基因表达.结果 (1)动脉粥样硬化组、稳定型心绞痛组和急性心肌梗死组血清氧化低密度脂蛋白抗体水平显著高于健康对照组(P<0.05);急性心肌梗死组血清氧化低密度脂蛋白抗体水平显著高于稳定型心绞痛组(P<0.05);稳定型心绞痛组与动脉粥样硬化组血清氧化低密度脂蛋白抗体水平差异无统计学意义(P>0.05);(2)急性心肌梗死组全血中基质金属蛋白酶-9高于动脉粥样硬化组、稳定型心绞痛组和健康对照组(P<0.05);动脉粥样硬化组、稳定型心绞痛组与健康对照组基质金属蛋白酶-9差异无统计学意义(P>0.05);动脉粥样硬化组与稳定型心绞痛组基质金属蛋白酶-9差异无统计学意义(P>0.05).结论与基质金属蛋白酶-9比较,氧化低密度脂蛋白抗体滴度升高对诊断动脉粥样硬化有一定的价值,特别对于动脉粥样硬化的早期阶段诊断有一定的优越性.Objective Toevaluate the clinical value ofantibodyagainst oxidized low-density lipoprotein in the diagnosis of atherosclerosis,compared with the matrix metallproteinase-9.Methods 30 patients with atherosclerosis,67 patients with coronary heart disease（included 36 patients with stable angina and 31 patients with acute myocardial infarction） and 34 healthy subjects were involved in this study.Serum levels of antibody against oxidized low-density lipoprotein were measured by enzyme-linked immunosorbent assay. Gene expression of matrix metallproteinase-9 was measured by RT-PCR.Results（1） Serum levels of antibody against oxidized low-density lipoprotein were significantly higher in patients with atherosclerosis,stable angina and acute myocardial infarction than those in healthy subjects（P0.05）;serum levels of antibody against oxidized low-density lipoprotein were significantly higher in patients with acute myocardial infarction than those in patients with stable angina（P0.05）;there was no statistical difference in serum levels of antibody against oxidized low-density lipoprotein between patients with stable angina and atherosclerosis（P0.05） .（2） Gene expression of the matrix metallproteinase-9 in patients with acute myocardial infarction was higher than it in healthy subjects and patients with atherosclerosis and stable angina（P0.05）;there was no statistical difference in gene expression of matrixmetallproteinase-9 between healthy subjects and patients with atherosclerosis or stable angina（P 0.05）;there was no statistical difference in gene expression of matrix metallproteinase-9 between patients with atherosclerosis and stable angina（P0.05） .Conclusion Compared with matrix metallproteinase-9,antibody against oxidized low-density lipoprotein has a certain value in the diagnosis of atherosclerosis,especially in early stage.
