摘 要:目的:观察椎动脉型颈椎病(CSA)动物模型血小板聚集功能的改变以及复方川脊片对其的影响。方法:采用硬化剂注射法复制CSA家兔模型,并用复方川脊片治疗,颈复康胶囊作对照,治疗4周后,行经颅多普勒超声(TCD)检查,聚椎动脉行HE染色,测定血管壁中膜厚度,同时采血检测血小板最大聚集率(MAR)。结果:模型组双侧椎动脉和基底动脉血液平均流速(Vm)均较空白组有不同程度降低(P<0.05),动脉的搏动指数(Pi)和阻力指数(Ri)均有不同程度增高(P<0.05);与模型组相比,预复康组和川脊片组上述指标均有不同程度改善(P<0.05),而川脊片组改善更明显。模型组椎动脉管壁中膜平滑肌细胞肥大增生明显,中膜较空白组明显增厚(P<0.05),颈复康组和川脊片组平滑肌细胞增生受到抑制,管壁中膜变薄。模型组血小板MAR较空白组明显增高(R<0.01),颈复康组和川脊片组血小板MAR较模型组明显下降(P<0.05),其中以川脊片组下降更加明显(P<0.01),但是2治疗组血小板MAR仍然明显高于空白组(P<0.05)。结论:复方川脊片可能是通过改善血小板聚集功能,抑制血管壁平滑肌细胞增殖,改善椎-基底动脉血供从而发挥治疗作用。Objective:To explore platelet aggregation changes in animal models of cervical spondylosis of vertebral artery type(CSA),and to investigate the effect of compound chuanji pills.Methods:CSA rabbit models were made by injecting sclerosing agent and then treated respectively with compound chuanji pills and Jingfukang capsules.After 4 weeks' treatment,transcranial doppler(TCD)was performed.Meanwhile,HE staining was done to test the tunica media thickness of vertebra artery and blood samples were taken to test platelet maximum aggregation rate(MAR).Results:Compared with blank group,the mean velocity(Vm)of bilateral vertebral artery and basal artery in model group was markedly decreased(P0.05)while pulsatility index(Pi)and resistant index(Ri)notably increased(P0.05).The above indexes were largely improved in the two treatment groups as compared with model group(P0.05).There was intensive proliferation of smooth muscle cells in model group and the tunica media of vertebra artery was apparently thicker compared to blank group(P0.05),but after treatment,proliferation of smooth muscle cells was strongly inhibited and the tunica media thickness was remarkably reduced(P0.05).Platelet maximum aggregation rate(MAR)in model group was significantly elevated compared to blank group(P0.01),while in the two treatment groups as compared to model group,MAR was dramatically declined(P0.05),with more obvious declination in compound chuanji pills group(P0.01).But MAR in the two treatment groups wastill higher than that in blank group(P0.05).Conclusion:The therapeutic effect of compound chuanji pills may be realized by improving platelet aggregation function and inhibit proliferation of smooth muscle cells which provide better blood supply for vertebral and basal artery.
