高龄股骨粗隆间骨折患者的术前风险评估及手术选择

摘　　要：目的:探讨高龄股骨粗隆间骨折患者的术前风险评估及手术方法的选择。方法:对70例高龄患者股骨粗隆间骨折的合并症、术前准备、手术适应症与内固定的选择、以及术后处理进行回顾性分析。结果:70例患者均顺利进行手术,住院期间出现并发症16例,死亡6例。术后随访58例(82.8%),根据患者骨折愈合、关节活动、主观感觉与生活能力恢复等因素评定疗效,优良率81.4%(57/70例)。结论:围手术期的全身状况评估、充分的术前准备,合理的麻醉和手术方式的选择以及术后处理和早期功髓锻炼是获得成功的关键。Objective：To explore the risk assessment and the choice of surgery before the operation in elderly patients with intertrochanteric femoral fracture.Methods：A retrospective analysis was performed in 70 aged patients with intertrochanteric femoral fracture.The complications,preoperative preparation,operative indications,internal fixation devices and postoperative management were reviewed.Results：All the patients＇ operations were successful.Sixteen cases showed complications,and six patients died.Fifty-eight cases were followed up.The treatment effect was evaluated by fracture healing,joint motion,patients＇ self-evaluation and ability to manage daily life,and excellent or good recovery was found in 57cases（81.4%）.Conclusion：There are several key factors to achieve satisfactory effect for elderly patients with intertrochanteric femoral fracture,such as accurate evaluation of general condition,sufficient preoperative preparation,reasonable anesthesia,appropriate choice of internal fixation devices,and treatment and early functional training after operation,etc.
