摘 要:目的:探讨低频超声心肌消融用以治疗心律失常的可行性及副作用。方法:以34KHz连续波超声作用于活体兔外膜心肌,记录工作中心肌细胞动作电位的变化及结构变化。结果:超声处理后心肌未见搏动及传导异常,经8W/cm超声消融后心肌细胞静息电位水平明显提高,0相最大除极速度及动作电位幅度明显降低。以26W/cm声强消融后跨膜动作电位消失。观察发现消融近端心肌凝固样坏死,内膜心肌(距外膜4~6mm)细胞器空化、破坏及肌丝溶解。结论:低频超声可有效消融心肌。Objective:Low frequency ultrasound was used to study the feasibility of ablation myocardial tachycardia.Method:Transmembrane potential of the working myocardium of rabbits was recorded after 34 KHz ultrasound treatment.The histology and ultrastructure effects were observed in the injured myocardium.Results:After 8w/cm 2 acoustic energy intensity ultrasound applied,the global and regional functions of the heart became normal,no arrhythmias and conduction block occurred.The rest potential of myocardial cell was significantly increased,the maximum depolarization velocity and action potential amplitude were significantly decreased.The action potential was disappeared by 26w/cm 2.Myocardial necrosis,near the ultrasound probe 2 3mm,cellular organs injured and destroyed.myofilaments dissolved at endocardial myocardium were all observed.Conclusion:Deep myocardial ablation can be induced by higher intensity low frequency ultrasound,being a new ablation energy for tachycardia therapy.By this technic,the potential basis of arrhythmia in injured myocardium can be ablated.
