摘 要:目的探讨实时三维超声心动图(RT-3DE)评价维持性血液透析(MHD)患者左心室收缩同步性的临床价值。方法对30例MHD患者(MHD组)和20例健康查体者(对照组)行RT-3DE检查,记录常规超声心动图参数,启动Fullvolume程序获取左心室全容积图像,用3DQ Advanced软件定量分析左心室收缩同步化运动指标。结果MHD组左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)均显著大于对照组(P<0.05或0.01),左室射血分数(LVEF)显著小于对照组(P<0.05),16节段、12节段(中间段和基底段)和6节段(基底段)最小收缩容积时间标准差、最大差值及心率校正值均显著大于对照组(P<0.05)。结论应用RT-3DE评价MHD患者左心室收缩同步化及心功能具有简便、可重复、无创性等优点,可为MHD患者病情评价及治疗指导提供依据。Objective To explore the clinical value of real-time three-dimensional eehocardiography(RT-3DE) on the evaluation of left ventricular systolic synchrony in patients with maintenance hemodialysis (MHD). Methods RT-3 DE examination were performed in 30 eases of MHD ( MHD group) and 20 normal subjects ( control group), the conventional echocardiography parameters were recorded, the Fullvolume program was used to acquire the left ventricular full-volume image ,the 3DQ advanced software was used to analyze the data of left ventricular systolic synehrony. Results The left ventrieular end diastolic volume ( LVEDV), left ventricular end systolic volume (LVESV) of the MHD group were significantly greater than those of the control group (P 〈0.05 or 0.01 ) ,the left ventfieular ejection fraction (LVEF) was significantly less than that of the control group (P 〈0.05 ), while the Tmax16-SD ,Tmax 16-Dif ,Tmsvl 2-SD ,Tmax 12-Dif ,Tmax 6-SD ,Tmax 6-Dif of the MHD group were significantly higher than those of the control group ( P 〈 0.05 ). Conclusions Application of RT-3DE to assess the MHD patients with left ventricular systolic synchrony and heart function has advantages of simple, repeatable, noninvasive and ere, that can provide basis for the evaluation and treatment of MHD patients.
