带涤纶环深静脉导管在维持性血液透析患者中的应用

摘　　要：目的探讨带涤纶套双腔静脉导管在维持性血液透析患者中的应用效果、并发症及处理。方法随访我院32例血液透析患者留置导管的使用情况、并发症,记录导管使用终点。结果共计32例患者导管置入后均正常使用,其中2例为颈外静脉切开置管,其余均为右颈内静脉置管。导管平均使用时间为(11.2±8.5)个月,最长时间为61个月。2例因导管感染拔除,1例因栓塞拔除,1例导管拔脱,4例因原发病死亡。结论带涤纶套双腔静脉导管可长期保留,感染率较低,血流量可以达到充分透析,为不能建立瘘管的患者解决血管通路问题。Objective To analyze the effect of PermCath dual lumen catheters in maintenance hemodialysis patients as well as the complications and treatment.Methods 32 patients with long-term vascular access for hemodialysis underwent implantation of PermCath dual lumen catheters for maintenance and the patients were followed up for the use,complications and the life span of the catheters.Results Indwelling of PermCath catheters were performed in 32 patients,with 30 catheters implanted in the right internal jugular vein and 2 catheters in the external jugular vein.The average indwelling time of the catheter was（11.2±8.5） months,and the longest dwelling lasted for 61 months.Failure of the catheterization was due to infections（2/32）,thrombosis formation（1/32） or detachment of the catheter（1/32）.Four patients died of uremia complications with normal function of their catheters.Conclusion PermCath catheter can be indwelled in veins for hemodialysis access for a long time with a relatively low incidence of infection,and the patients could get adequate blood flow with dialysis.It provides routine vascular access for the patients with failure of fistula establishment.
