彩色多普勒超声在弥漫性胰腺癌中的应用

摘　　要：目的应用彩色多普勒超声观察弥漫性胰腺癌结构及血流变化,探讨其声像特征。方法应用彩色超声多普勒诊断仪,探头频率2.5～5MHz,经穿刺或剖腹探查组织活检,观察其胰腺大小、形态、结构、内部血流变化,及其周围脏器、腹腔、腹膜后有无异常。结果 21例胰腺癌患者光点均粗糙,分布明显欠均,其中9例(42.9%)表面略显示凹凸不平;15例(71.4%)胰腺癌患者整个实质为低回声,6例(28.6%)回声变化不明显;3例(14.3%)胰腺癌患者可见细小短线状血流,18例(85.7%)为星点状血流;少部分出现周围脏器的侵犯,淋巴结转移及腹水等。结论弥漫性胰腺癌形态无特殊变化,内部结构及血流发生不同程度改变,有其明显的声像特征。Objective To observe the structure of diffuse pancreatic cancer and the changes of blood flowing through color Doppler ultrasound,and to explore ultrasonographic characteristics of the disease.Methods With the probe frequency of 2.5～5 MHz,color Doppler diagnostic apparatus was used to observe the size,morphology,structure,internal blood flow,organs around the pancreas,the abdominal cavity and retroperitoneal alteration.Results Twenty-one patients with pancreatic cancer had rough spots and unclear distribution,of which 9 （42.9%） patients showed slightly uneven surface,15 （71.4%） patients showed hypo echoic patterns,6 patients （28.6%） showed no significant change in echo.3 patients （14.3%） showed a small short-term blood flow,and 18 patients （85.7%） showed scattered color Dopper flow signal.Violation of the surrounding organs,lymph node metastasis and ascites were found in a few patients.Conclusions No specific change was observed in morphology of diffuse pancreatic cancer,but the internal structure and blood flow were changed in different degrees,and with obvious ultrasonographic characteristics.
