摘  要:目的探讨维生素D受体(VDR)和甘露糖结合蛋白(MBP)基因多态性及其交互作用与汉族成年人肺结核发病的关系。方法选择汉族成年人新发肺结核患者182例和成年健康体检者190例组成病例组和对照组。调查研究对象的一般情况,分别以PCR-RFLP、特异引物PCR方法分析VDR和MBP基因多态性。结果病例组和对照组VDR基因Taq Ⅰ、Apa Ⅰ、Fok Ⅰ及MBP基因P52、P54位点突变型等位基因频率分别为4.0%、29.1%、49.2%、8.6%、6.7%和6.1%、28.2%、46.1%、3.9%、6.3%。携带P52突变型基因为肺结核发生的危险因素。FokⅠFf与ApaI Aa、Fok Ⅰ Ff与P52突变基因型之间存在交互作用,OR及其95% CI分别为2.170(1.198~3.931)和6.063(1.324~27.756)。同时携带Fok Ⅰ Ff与ApaI Aa者发生肺结核的危险性提高2.5倍。结论 VDR基因Fok Ⅰ杂合子与ApaI杂合子或MBP基因P52突变型之间均存在协同作用。Objective To study the relationship of the genetic polymorphisms of Vitamin D receptor(VDR) and Mannose binding protein (MBP) and susceptibility to pulmonary tuberculosis and the gene-gene interaction in a Chinese Population in Tangshan area.Methods One hundred and eighty-two cases and 190 controls were collected and a case-control study was conducted.Information on related risk factors of tuberculosis was collected.Polymerase chain reaction with restriction fragment length polymorphism(PCR-RFLP)and sequence-specific primers(PCR-SSP)were carried out in the polymorphism in VDR and MBP gene, respectively.The Fok Ⅰ, Taq Ⅰ and Apa Ⅰ enzyme were employed.Results The frequency of mutant allele of VDR and MBP gene on Fok Ⅰ, Taq Ⅰ, Apa Ⅰ,P52 and P54 were 4.0%,29.1%,49.2%,8.6%,6.7% and 6.1%,28.2%,46.1%,3.9%,6.3% in cases and controls, respectively.The univariate and multivariate logistic analyses demonstrated that P52 gene mutant were risk factors to pulmonary tuberculosis.The interaction was confirmed on Fok Ⅰ Ff and ApaI Aa,Fok Ⅰ Ff and P52 mutant with OR(95%CI)being 2.170(1.198-3.931) and 6.063(1.324-27.756) respectively.Who held Fok Ⅰ Ff and ApaI Aa will be likely to induce pulmonary tuberculosis, the risk increased 2.5 times.Conclusion The synergetic effect is proved on susceptibility to pulmonary tuberculosis between Fok Ⅰ Ff and ApaI Aa,Fok Ⅰ Ff and P52 mutant genetype.
