摘  要:目的:探讨心磁图(MCG)在冠心病(CHD)诊断中的临床应用价值。方法:应用美国CardioMagImaging公司生产的CMI-2409型心磁图仪,分别对383例冠心病患者(冠状动脉造影显示至少1支冠状动脉主要分支狭窄≥50%)和110例非冠心病患者(冠状动脉造影无狭窄)进行心磁图检查,对MCG的7项参数T峰前磁向量、T峰前磁移动距离、T峰前磁向量变异率、T峰后磁向量、T峰后磁移动距离、T峰后磁向量变异率、T峰前后磁向量变化进行分析。结果:(1)冠心病组与非冠心病组MCG7项参数差异均具有显著性意义(P<0.01)。(2)T峰前磁向量、T峰前磁移动距离、T峰前磁向量变异率、T峰后磁向量、T峰后磁移动距离、T峰后磁向量变异率、T峰前后磁向量变化诊断冠心病的敏感性分别为40.73%、51.17%、50.39%、30.81%、52.74%、55.87%、31.59%,特异性分别为83.64%、71.82%、73.64%、90.91%、70.91%、71.82%、97.27%。MCG7项参数中任意3项以上阳性,诊断冠心病的敏感性为56.40%,特异性为90.91%。结论:MCG是一项无创性、对冠心病诊断有较高价值的检查方法。Objective: To evaluate the clinical application of magnetocardiography (MCG) in diagnosis of coronary heart disease (CHD). Methods: MCG measurements were performed in 383 patients with CHD (coronary angiography showed that at least one major branch of coronary artery stenosis t〉 50%)and 110 patient with non-CHD (coronary arteriography negative) by CMI-2409 MCG instrument produted by CardioMag Imaging Company. Seven parameters of MCG were nanlyzed pre-peak repolarization angle, pre-peak repolarization trajectory, pre-peak repolarization angular deviation, post-peak repolarization angle, post-peak repolarization trajectory, post-peak repolarization angular deviation and pre-post angle change. Results: (i) Significant difference were found in seven parramenters of MCG between CHD and non-CHD group. (2) The diagnostic sensitivity of pre-angle, pre-trajectory, pre-angular deviation, post- angle, post-trajectory, post-angular deviation, pre-post angle change were 40.73%, 51.17%, 50.39%, 30.81%, 52.74%, 55.87%, 31.59% respeetively, the specificity were 83.64%, 71.82%, 73.64%, 90.91%, 70.91%, 71.82%, 97.27% respectively. Three or more paramenters were positive, the sensitivity for diagnosis of coronary heart disease was 56.40% and specificity was 90.91%. Conclusion: MCG is a noninvasive, higher values method for the diagnosis of CHD.
