老年糖尿病患者血糖波动与C反应蛋白的相关性研究

摘　　要：目的探讨老年糖尿病患者血糖波动与C反应蛋白（CRP）的关系。方法采用动态血糖监测系统（CGMS）对98例老年2型糖尿病患者进行持续3d血糖监测，读取CGMS监测期间平均血糖值（MBG）及其标准差（SD），以3SD〈MBG为血糖波动小组，3sD≥MBG为血糖波动大组，比较两组问生化指标及CRP水平的差异。结果血糖波动大组与血糖波动小组CGMS监测期间的MBG相比有差异但无统计学意义[（7．9±1．8）mmol／LVS（7．3±1．4）mmol／L，P=0．070】，但血糖波动大组CRP水平较高，与血糖波动小组比较差异有统计学意义[1．21（0．39-4．90）坩0．76（0．46～1．14），P=0．035】。相关分析表明CRP与SD呈正相关（r=0．42，P=0．032）。结论血糖波动与CRP水平有关，血糖波动大的患者的CRP水平较高，血糖波动大可能参与老年2型糖尿病患者炎症反应及氧化应激。Objective To investigate the relationship between blood glucose fluctuations and c-reactive protein （CRP） in elderly type 2 diabetic patients. Methods 98 elderly type 2 diabetic patients were recruited in the study, and were under- went the evaluation of the mean blood glucose value （MBG） and standard deviation （ SD ） by continuous glucose monitoring system （CGMS） for 3 days. According to the relationship between 3SD and MBG, 98 cases were divided into two groups： higher blood glucose fluctuations group （3SD~〉MBG） and lower blood glucose fluctuations group （3SD 〈 MBG）. The CRP levels of the two groups were assessed and compared. Results There was no difference in the MBG between the higher blood glucose fluctuations and lower blood glucose fluctuations groups [（7.9± 1.8 ） mmol / L vs （ 7.3± 1.4 ） mmol/L, P = 0.070]. Whereas the levels of CRP in the higher blood glucose fluctuations group significantly increased, as compared with those in lower blood glucose fluctuations group [1.21 （0.39-4.90） vs 0.76 （0.46-1.14）, P = 0.035]. The level of CRP was positive correlation with SD. Conclusions Blood glucose fluctuations are associated with CRP level in elderly type 2 diabetic patients. Higher blood glucose fluctuations lead to higher CRP. Over blood glucose fluctuations involve higher oxidative stress in the elderly patients with type 2 diabetes.
