高血压冠状动脉钙化与视网膜血管改变的相关性

摘　　要：目的:探讨高血压患者视网膜动脉狭窄程度与冠状动脉钙化的相关性。方法:对98例高血压患者,分别通过眼底照相和双源64层螺旋CT冠状动脉造影,进行视网膜动脉狭窄程度、动静脉交叉压迫征和冠状动脉钙化积分的分析,并应用多因素分析方法 ,探讨与视网膜动脉狭窄程度相关的危险因素。结果:出现视网膜动脉狭窄和动静脉交叉压迫征的高血压患者,冠状动脉总钙化积分明显增加(P<0.01)。并且,随着视网膜动脉狭窄程度和动静脉交叉压迫征严重度的增加,冠状动脉总钙化积分亦逐渐增加(P<0.05)。Logistic回归分析,发现视网膜动脉狭窄与高血压病程、舒张压、甘油三酯、胆固醇和冠状动脉总钙化积分有关,经调整高血压病程、舒张压、甘油三酯、胆固醇等因素后,视网膜动脉狭窄与冠状动脉总钙化积分相关。结论:高血压视网膜动脉狭窄程度与冠状动脉钙化具有相关性。Objective To explore the relationship between coronary artery calcium quantification and retinal vasculopathy in hypertension patients.Methods Fundus images and 64-slice CT coronary artery angiography were taken in 98 hypertension patients without diabetes.The changes of hypertension retinopathy including retinal arteriolar focal narrowing,arteriovenous nicking,and coronary artery calcium quantification were analyzed.Results The coronary artery calcium quantification was significantly increased with retinal arteriolar focal narrowing and arteriovenous nicking（P0.01）in hypertension patients.The more retinal arteriolar focal narrowing and arteriovenous nicking became,the more coronary artery calcium quantification was（P0.05）.Logistic regression analysis indicated that the grade of retinal arteriolar focal narrowing was related to the length of hypertension history,the level of diastolic blood pressure,triglyceride,cholesterol,and the score of coronary artery calcium.After adjusting history of hypertension,diastolic blood pressure,triglyceride and cholesterol,the grade of retinal arteriolar focal narrowing was correlated to the score of coronary artery calcium.Conclusions The grade of retinal arteriolar focal narrowing was correlated with the score of coronary artery calcification.
