无菌切口手术患者不合理使用抗菌药物对住院费用的影响

摘　　要：目的了解无菌手术切口患者预防性使用抗菌药物情况,探讨如何规范用药后可降低医疗费用。方法对某医院2008年664例无菌手术切口患者的用药情况进行调查。结果无菌手术切口患者抗菌药物使用率为98.2%。纠正不合理用药后,人均节省费用641元,占出院总费用的5.3%。随着年龄的增高,合理用药可节省费用也有增高趋势。脑外科手术患者合理用药可节省费用明显高于其他科室,与其病情严重程度有关。中、高级职称医师合理用药可节省费用远高于初级职称医师。病情越复杂,合理用药可节省的费用也就越多。皮肤和皮下组织手术实际抗菌药物费用最高,费用调整用药后可节省费用最多。结论该院无菌切口手术患者抗菌药物使用不合理,给患者住院费用造成极大的浪费。选择正确的给药方案,使临床用药真正做到安全、经济、有效、合理和经济。Objective To understand the situation of patients of the sterile surgical operation in preventive use of antimicrobial drug,and study how to reduce the medical costs after treatment.Methods In 2008,664 cases of aseptic surgical incision of the patient＇s drug investigation were researched.Results The using rate of antibacterial agent in sterile incision operation was 98.2%.After correcting the irrational using of drugs,the per capita will save costs 641 yuan,count 5.3% of total cost of hospital.As the age increased,the rational using of medicine may save more.The brain surgery patients to use drugs rationally will more saver than the other department,and its serious extent.The senior doctor may save expenses rational using of medicinefar above the junior doctor.As illness was complicated as more rational using of medicine cound reduce.Antimicrobial drug cost of skin and hypoderma operation was most,and saving cost after adjustment in medicine was most.Conclusion The surgical incision in patients with sterile hospital antibiotic drug used unreasonable,hospital charges to patients caused great waste.Therefore,selects the correct dosage regimen,clinical medicine really safe,economical,effective,rational and economically with the more great economic significance.
