冠状动脉介入患者股动脉穿刺点三种不同止血方法效果比较及护理

摘　　要：目的:探讨冠状动脉造影及冠状动脉介入治疗(PCI)患者股动脉穿刺点三种不同止血方法的临床效果及护理方法。方法:将396例行冠状动脉造影及PCI患者按股动脉穿刺点止血方法不同分为手工压迫组162例、Femostop组106例及Perclose组128例。评价各组止血操作时间,出血、血栓事件发生率,术后舒适性相关指标的差异。结果:Femostop组与Perclose组止血操作时间与手工压迫组相比均缩短(P<0.01),Perclose组出血发生率均低于Femostop组、手工压迫组(P<0.01),Perclose组术后舒适性各指标均优于手工压迫组(P<0.01),Femostop组术后舒适性各指标与手工压迫组比较差异无统计学意义(P>0.05)。结论:Perclose缝合器缝合止血法与Femostop器械压迫止血法均可缩短压迫止血操作时间;Perclose缝合器缝合止血法可降低术后出血发生率,改善术后患者舒适程度。Objective： To explore the clinical effect of three different kinds of hemostasis for femoral artery puncture point in the patients with coronary angiography and percutaneous coronary intervention（PCI） and nursing methods.Methods： 396 patients who underwent coronary angiography and PCI were divided into manual pressure group（n=162）,Femostop group（n=106） and Perclose group（n=128） according to the adoption of different hemostasis.The operation time of hemostasis,incidence of bleeding and thrombosis and the difference of related indices of postoperative comfort were evaluated in each group.Results： The operation time of hemostasis was shorter in Femostop and Perclose groups compared with that in manual pressure group（P〈0.01）;the incidence of bleeding was lower in Perclose group compared with that in Femostop and manual pressure groups（P〈0.01）;there was no statistically significant difference in the comparison of related indices of postoperative comfort between Femostop group and manual pressure group（P〉0.05）.Conclusion： Both Perclose stapler suture hemostasis and Femostop device compression hemostasis can shorten the operation time,and Perclose stapler suture hemostasis can reduce the incidence of postoperative bleeding and improve patient＇s comfort after surgery.
