医生与家长合作早期干预治疗新生儿缺氧缺血性脑损伤的临床研究

摘　　要：目的:探索在我国目前的国情下,早期干预治疗新生儿缺氧缺血性脑损伤(HIBD)的模式。方法:选取140例中重度HIBD患儿,以出生顺序半随机分成早期干预组和对照组。对早期干预组采用不断评估-干预的程序由医生和家长共同完成训练计划至2岁,医生制定训练计划,培训家长并对每个患儿进行评估。家长负责运动、认知、语言和社会交往能力等具体训练计划的执行。采用生长发育指标、临床、CT资料和发育商(DQ)、智力指数(MI)评价治疗效果。结果:干预组患儿6月、12月、18月、24月龄时的DQ、MI值明显高于对照组(P<0.01)。干预组的临床和CT预后不良率明显低于对照组(P<0.01,P<0.05)。结论:采用医生和家长合作模式进行的早期干预治疗是可行的,能有效地防止和减轻HIBD患儿的后遗症,减少伤残,改善预后。Objective： To explore the mode of early intervention for children with hypoxic ischemic brain damage （ HIBD） under the current condition of China. Methods： 140 children with moderate and severe HIBD were divided into early intervention group and control group randomly according to the sequence of birth,the children in early intervention group received continuous evaluation - intervention procedure by the cooperation of doctors and parents until two years old; the training programs were established by doctors,and the parents were trained to evaluate the children and carried out the treatment of motion,cognition,language and social intercourse of the children. The therapeutic effects were evaluated by growth and development index,clinics,computer tomography （ CT） ,development quotient （ DQ） and mentality index （ MI） . Results： At 6,12,18 and 24 months,the DQ and MI in early intervention group were significantly higher than those in control group （ P 0. 01） . The poor prognosis rates of clinics and CT in early intervention group were significantly lower than those in control group （ P 0. 01,P 0. 05） . Conclusion： Early intervention treatment by cooperation of doctors and parents is feasible,which can prevent and relieve the sequelae of children with HIBD effectively and diminish disability,thus improve prognosis.
