摘  要:目的对胎儿心脏进行二维及实时三维超声成像并进行对比,探讨实时三维超声成像的应用价值。方法对129例孕24~40周的正常胎儿进行超声心动图检查,以二维超声观察胎儿心脏各切面,包括:四腔心观、左室流出道长轴观、右室流出道长轴观、大动脉短轴观、主动脉弓长轴观、动脉导管弓长轴观、三血管短轴观、三血管长轴观等,并应用实时三维成像,以一定的角度进行切割,从而获取上述各切面,观察相应的结构。结果除四腔心观和三血管短轴观外,其余切面观三维均较二维成像显示率高,二维及实时三维超声显示满意例数分别为113例和122例,差异显著(P〈0.05)。结论实时三维超声能获取较二维超声更多的胎儿心脏结构信息,能获得二维成像困难或无法观察的切面,实时三维超声心动图提供了一个胎儿心脏检查的新视窗。Objective To compare the capability between two-dimensional echocardiography (2-DE) and three dimensional realtime echocardiography (3-DRTE) in visualizing the fetal hearts and to evaluate the applied value of 3-DRTE. Methods A hundred and twenty-nine normal fetus with pregnant age for 24-40 weeks initially underwent 2-DE examinations which comprised the observations of atria, ventricle and junction of the both, atrial-ventricular valve and septum, oval and its valve, arterial duct, aorta, pulmonary artery, superior vena cava and its relationship with cardiac junction, inside diameter and line feed of aortic arch etc and then using 3-DRTE acquired each section by certain angle cutting and observed each structure above mentioned. Results The satisfactory cases showed by 2-DE and 3- DRTE were 113 and 122, respectively, and there was significant difference between the both (P 0.05). Conclusion 3-DRTE in providing the information of fetal heart structures is significantly superior to 2-DE, especially 3-DRTE could visualize some fine structures which could not be found by 2-DE, so that 3-DRTE represents a new approach to feteal heart imaging.
