单一免疫法与联合免疫法检测大便潜血的临床应用评价

摘　　要：观察两种大便潜血实验方法—单一免疫法(胶体金单克隆抗体血红蛋白检测法)和联合免疫法(血红蛋白、转铁蛋白联合免疫法)检测的灵敏度、特异性和抗干扰性,以便在工作中避免假阳性和假阴性结果。用单一免疫法和联合免疫法检测人7种不同浓度血红蛋白液,对比其灵敏度。取人经常食用的6种动物血红蛋白检验单一免疫法和联合免疫法的特异性和准确度。比较单一免疫法和联合免疫法对107例经内窥镜检查确诊消化道出血患者的阳性检出率。结果显示,单一免疫法和联合免疫法的灵敏度和特异性相同,联合免疫法的稳定性和抗干扰性优于单一免疫法,单一免疫法可出现假阴性和假阳性反应。联合免疫法是目前检测大便潜血的较好的方法,将转铁蛋白作为补充检测手段是临床判断是否存在隐血的较有价值的方法。To compare the sensitivity and specificity of hemoglobin colloidal gold assay and hemoglobin and transferrin united immunoassay in detecting fecal occult blood,seven different concentrations of human hemoglobin and six different kinds of animal hemoglobin were measured with colloidal gold assay and united immunoassay respectively to check the sensitivity and specificity of assay.These two immunoassays were used to detect dung occult blood in 107 patients with alimentary canal diseases.The results showed that the hemoglobin colloidal gold assay and the hemoglobin and transferrin united immunoassay had same sensitivity and specificity.The united immunoassay had higher ability of anti-interference and specificity than that of colloidal gold assay.The hemoglobin and transferrin united immunoassay could be a good method in detecting fecal occult blood.
