摘  要:目的探讨热休克蛋白(HSP)70-1+190(G/C)、HSP70-2+1267(A/G)位点基因多态性与煤尘接触工人早期肺部改变的相关性。方法采用病例-对照研究,随机选取河南省某煤矿的工人210例,其中86例胸片结果判定为"观察对象"的为病例组,124例胸片结果判定为正常的为对照组。应用聚合酶链反应-限制性片段长度多态性分析(PCR-RFLP)技术检测HSP70-1+190(G/C)和HSP70-2+1267(A/G)的单核苷酸多态性。结果观察对象与同一煤矿胸片结果显示正常的工人HSP70-1+190(G/C)位点的3种基因型GG、GC和CC及G和C等位基因频率在病例组和对照组之间的分布,差异均有统计学意义(2=13.93,P=0.00;2=8.24,P=0.00),CC基因型个体发生早期肺部改变的风险是GG基因型个体的2.42倍(OR=2.42,95%CI:1.20~4.89)。HSP70-2+1267(A/G)位点的基因型及等位基因的分布在2组之间差异均无统计学意义(P>0.05)。结论 HSP70-1+190(G/C)位点多态性可能与煤尘接触工人早期肺部改变相关,携带CC基因型煤尘接触工人更易发生早期肺部改变。Objective To investigate the relationship among heat shock protein 70(HSP70) gene polymorphism and early changes in lungs of coal worker.Methods A case-control study was conducted in 86 workers who were assessed to be "the observation objects" by chest radiography (the case group) and in 124 workers was assessed to be "the normal ones" by chest radiography (the control group).Polymerase chain reaction-restrained fragment length polymorphism technique (PCR-RFLP) was applied to detect the single nucleotide polymorphisms (SNP) of HSP70-1+190(G/C) and HSP70-2+1267(A/G).Results There were significant differences in the distribution of 3 genotypes GG,GC,CC and 2 alleles G and C frequencies of HSP70-1+190(G/C) between the case and the control group (X2=13.93,P=0.00;X2=8.24,P=0.00) and the risk of CWP in the individuals with CC genotype was 2.42 folds higher than that with GG genotype (OR=2.42,95%CI:1.20-4.89).There were no significant differences in the distribution of genotypes and allele frequencies of HSP70-2+1267(A/G) between the case and the control group (P0.05).Conclusion HSP70-1+190(G/C) polymorphism might be associated with the susceptibility to early changes in lungs and the individuals with the CC genotype might have more tendency than the others.
