摘 要:目的:探讨重组腺病毒低氧诱导因子-1α(adenovirus mediated hypoxia-inducible factor-1α,Ad-HIF-1α)对兔急性缺血后肢血流灌注情况的影响。方法:20只新西兰大白兔建立急性后肢缺血模型,随机分成3组,分别在术后即刻肌注生理盐水(NS组,n=6)、腺病毒空载体(Ad-null组,n=6)和Ad-HIF-1α(Ad-HIF-1α组,n=8)。术后10d行Real-time PCR检测外源基因在转染部位骨骼肌的表达情况;对比超声(contrast enhanced ultrasound,CEU)检查了解Ad-HIF-1α对缺血局部血流灌注的改善情况;伊文思兰法对缺血局部新生血管通透性进行评价。结果:基因转染后10d,Real-time PCR可检测到外源HIF-1α在转染局部骨骼肌的表达;CEU结果示Ad-HIF-1α组血流灌注优于Ad-null组和NS组(P<0.01);伊文思兰法检测3组新生血管通透性相似,定量评价差异无统计学意义(P>0.05)。结论:Ad-HIF-1α能改善缺血肢体的血流灌注,未明显增加新生血管的通透性。Objective To investigate the effects of adenovirus mediated hypoxia-inducible factor-1α(AdHIF-1α) gene on the blood perfusion in a rabbit model of ischemic hindlimb.Methods After ligation of left femoral artery, 20 New Zealand white rabbits were randomly divided into 3 groups:saline group(n = 6), Ad-null group(n = 6) and Ad-HIF-1α group(n = 8), and then received saline, Ad-null or Ad-HIF-1α respectly by intramuscular injection.10 days after the operation, contrast enhanced ultrasound(CEU) was used to observe the blood perfusion of the ischemic hindlimb.The permeability of the neovasculature in the ischemic skeletal muscle was evaluated by Evans blue assay.Results On the 10th day after gene transfection, HIF-1α gene expression in skeletel muscle was detected by Real-time PCR.CEU revealed better blood perfusion in Ad-HIF-1α group than the group treated with saline or Ad-null(P 0.01).Quantification of the leakage levels of Evans blue dye demonstrated that there were no obvious differences among three groups in neovascular permeability(P 0.05).Conclusion Ad-HIF-1α can improve blood perfusion of ischemic limb and did not deteriorate vessel leakage.
