痰热清注射液雾化吸入治疗呼吸系统疾病的系统评价

摘　　要：目的:系统评价痰热清注射液雾化吸入治疗呼吸系统疾病的疗效与安全性。方法:计算机检索Cochrane图书馆、MEDLINE、CBM、CNKI、万方数据库、维普资讯网发表的痰热清注射液雾化吸入治疗呼吸系统疾病的随机对照试验,按纳入标准纳入合格的随机对照研究并对其进行质量评价,采用RevMan4.2.2软件进行统计分析。结果:共纳入16项研究,根据对照药物不同分为6组,对于症状改善有效率的Meta分析结果显示,空白对照组OR=2.58,95%CI(1.68,3.96),并比较了症状改善时间,布地奈德组OR=1.15,95%CI(0.42,3.12);庆大霉素组OR=2.82,95%CI(1.65,4.82);糜蛋白酶组OR=2.36,95%CI(0.63,8.92);庆大霉素+糜蛋白酶组OR=3.03,95%CI(1.86,4.96);庆大霉素+糜蛋白酶+地塞米松组OR=3.14,95%CI(0.68,14.50),并比较了症状改善时间。结论:根据目前获得的低质量研究,治疗呼吸系统疾病方面,在西医常规治疗的基础上,痰热清注射液雾化吸入疗效优于空白对照、庆大霉素、庆大霉素+糜蛋白酶雾化吸入;而与布地奈德雾化混悬液、糜蛋白酶、庆大霉素+糜蛋白酶+地塞米松疗效相当。在症状消失时间方面,痰热清注射液雾化吸入可明显改善患者咳嗽、气喘、肺部啰音等症状。OBJECTIVE：To evaluate the efficacy and safety of Tanreqing Injection（aerosol inhalation） in the treatment of the respiratory system diseases.METHODS：The Cochrane Library,MEDLINE,CBM,CNKI,Wanfang data,CBM,CQVIP were searched for randomized control trials（RCTs） of Tanreqing Injections（aerosol inhalation） in the treatment of the respiratory system diseases.The quality of included RCTs that met the criteria recommended was assessed.Statistical analyses were performed using Revman 4.2.2 software.RESULTS：Sixteen RCTs were included and which were divided into 6 groups according to different control drugs.Meta-analyses on symptom improvement rate showed thatOR = 2.58 with 95% CI（1.68,3.96） for normal control group,OR = 1.15 with 95% CI（0.42,3.12） for Budesonide Suspension for Inhalation Group,OR = 2.82 with 95% CI（1.65,4.82） for Gentamycin for Injection Group;OR = 3.03 with 95% CI（1.86,4.96） for Gentamycin plus Chymotrypsin for Injection Group;OR = 3.14 with 95% CI（0.68,14.50） for Gentamycin in combination with Chymotrypsin and Dexamethasone Group.The time to achieve symptom improvement among all groups was recorded.CONCLUSION：It has been reported by current low quality RCTs on the treatment of the respiratory system diseases that Tanreqing Suspension for Inhalation in combination with routine western therapy is superior to blank control,Gentamycin,and Gentamycin in combination with Chymotrypsin,but equivalent to Budesonide Suspension for Inhalation,Chymotrypsin,and Gentamycin concomitant with Chymotrypsin and Dexamethasone in curative efficacy.Regarding symptom relief time,Tanreqing Injections remarkably improve patients＇symptoms including cough,panting and rales etc.
