复方苦参注射液对改善晚期大肠癌不同证型生存质量的临床研究

摘　　要：目的:评价复方苦参注射液对改善晚期大肠癌不同证型生存质量的影响。方法:我院2006年6月～2009年12月收治的晚期大肠癌患者,据临床表现,将其归纳为湿热蕴结、气滞血瘀和脾肾、气血亏虚之虚实两组。实证组70例,虚证组76例,予相同的支持、对症等常规西医治疗和复方苦参注射液30mL静脉滴注。结果:复方苦参注射液应用于实证组在近期疗效及生存质量、血便改善方面优于虚证组,而在缓解疼痛方面两组没有显著差异。结论:复方苦参注射液对于改善大肠癌患者生存质量具有一定的效果,其中对于中医辨证分型为湿热蕴结、气滞血瘀的患者效果更佳,而且用药安全,值得临床推广应用,并提倡辨证使用中成药,以取得更佳的临床疗效。OBJECTIVE:To evaluate the effects of compound matrine injection on the quality of life in patients with advanced colorectal carcinoma(in different types of syndromes).METHODS:The patients with advanced colorectal carcinoma admitted from June 2006 to Dec.2009 were divided into two groups according to their different clinical manifestations,i.e.,sthenia syndrome group(dampness and heat integration with QI-stagnancy and blood stasis,n = 70) andasthenia syndrome group(deficiency of both Spleen and Kidney,asthenia of both Qi and blood group,n = 76).Both groups were given the same supportive and symptomatic treatments(routine western therapy) plus intravenous drip infusion of compound matrine injection at a dose of 30ml per day.RESULTS:Sthenia syndrome group showed a more satisfactory outcome than didasthenia syndrome in short-term curative efficacy,improvement of quality of life andbloody stool;however,there were no significant differences between the two groups in pain relief.CONCLUSION:The compound matrine injection has certain efficacy for improving the quality of life in patients with advanced colorectal carcinoma,much as in those patients presenting with such syndrome as dampness and heat integration with QI-stagnacncy and blood-stasis.Besides,compound matrine injection is safe to use and thus worthy of being recommended in clinical practice.It is proposed to use this kind ofChinese patent medicine bydifferentiatingsymptoms and signs so asto achieve a better clinical efficacy.
