异氟醚或异氟醚复合拉贝洛尔控制性降压对肺内分流的影响

摘  要:研究了异氟醚或异氟醚复合拉贝洛尔控制性降压对肺内分流的影响。24例择期颅脑手术患者,随机分成两组:单纯异氟醚降压组和异氟醚复合拉贝洛尔降压组。患者的平均动脉压降低30%左右,分别于控制性降压前、降压达预定值时、维持降压20min时和停止降压20min后抽取患者动脉血和混合静脉血进行血气分析,采用标准公式计算肺内分流。结果显示,降压期间两组患者的肺内分流较降压前有所增加,但无统计学意义。由此认为,异氟醚或异氟醚复合拉贝洛尔控制性降压对肺内分流的影响较小。The effects of isoflurane with or without labetalol on intrapulmonary shunting were studied during induced hypotension. Twenty four ASAⅠ-Ⅱ patients who required deliberated hypotension for selective cerebral surgery were randomly divided into two groups( n =12 in each group), groupⅠ(isoflurane alone) and groupⅡ(isoflurane plus labetalol). Mean arterial pressure was reduced by approximately 30 %. Arterial and mixed venous blood gas samples were respectively collected and analyzed at four time points to determine right to left shunt fractions(Qs/Qt): 1)immediate before the start of hypotension ( baseline prehypotension), 2) goal hypotension was obtained,3)20 min after stable goal hpotension and 4) 20 min after the end of hypotension. The results showed that Qs/Qt during induced hypotension in the two groups had a slight increase from baseline prehypotension with the difference not being statistically significant. It was indicated that the effects of isoflurane with or without labetalol on Qs/Qt may be slight during induced hypotension.
