GPI联合RF检测在类风湿关节炎中的诊断价值

摘　　要：目的检测6-磷酸葡萄糖异构酶(glucose-6-phosphateisomerase,GPI)抗原在类风湿关节炎(Rheumatoid Arthritis,RA)中的表达情况,并探讨其与类风湿因子(rheumatoid factor,RF)联合检测在RA中的诊断和鉴别诊断价值。方法采用酶联免疫吸附试验(ELISA)检测65例RA患者、62例非RA患者血清中GPI抗原的浓度,采用免疫比浊法检测每位患者血清中类风湿因子(RF)的滴度。比较RA组和非RA组GPI抗原水平,分析GPI与RF的相关性。结果 GPI抗原对RA诊断的敏感性为44.6%,特异性为93.5%;RF的敏感性为72.3%,特异性为75.8%;GPI联合RF检测的敏感性为41.5%,特异性为96.8%。RA组GPI抗原水平高于非RA组(P<0.001),GPI抗原与RF存在正相关性(P<0.001)。结论 GPI特异性高,敏感性低,可用于RA的特异性诊断及与其它疾病的鉴别诊断,而GPI抗原与RF联合检测可提高RA诊断的特异性。Objective To examine the expression of GPI in rheumatic arthritis（RA）and explore the diagnostic value of the joint detection of RF with GPI.Methods The level of serum GPI in 65 patients with RA,62 patients with other diseases were detected by ELISA,and the serum RF were detected by immunoturbidimetry.The level of GPI between RA and non-RA patients was compared,and the correlation among the level of GPI and RF were analyzed.Results In RA,the sensitivity of GPI was 44.6% and the specificity was 93.5%;the sensitivity of RF was 72.3% and the specificity was75.8%;the sensitivity of the joint detection of RF with GPI was 41.5% and the specificity was 96.8%;the level of GPI in RA group was higher than non-RA group（P〈0.001）,it has correlation between GPI and RF（P〈0.001）.Conclusion GPI has high specificity but low sensitivity,and can be used for specific and discriminate diagnosis,and the joint detection of RF with GPI raised the specificity of diagnosis in RA.
