摘  要:将病例分为绒毛膜羊膜炎组和非绒毛膜羊膜炎组,采用免疫组化SP法检测32例胎膜早破(PROM)和20例正常孕妇胎膜组织中核转录因子κB(NF-κB)、环氧合酶-2(COX-2)及基质金属蛋白酶-9(MMP-9)的表达。结果表明:所有PROM胎膜标本,诊断为绒毛膜羊膜炎者占46.88%,正常组中胎膜感染占10.00%;PROM组胎膜组织中NF-κB、COX-2、MMP-9表达明显高于对照组孕妇(P<0.05);绒毛膜羊膜炎组胎膜NF-κB、COX-2、MMP-9表达明显高于非绒毛膜羊膜炎组(P<0.05);胎膜早破组患者胎膜组织中COX-2、MMP-9阳性表达与NF-κB采用线性相关分析呈明显正相关关系(P<0.01)。NF-κB、COX-2及MMP-9均参与了PROM的发病。COX-2、MMP-9基因的表达受多功能NF-κB的调控。NF-κB活化可能是PROM发生的关键因素。A piece of membrane was immediately collected after labor, HE stain. After diagnosing chorioamnionitis, all cases were divided into two groups: chorioamnionitis group and non-chorioamnionitis group. We used immunohistochemical method to assess the expression of nuclear factor kappa (NF-κB), cyelooxygenase-2 (COX-2) and matrix metal- loproteinase-9 (MMP-9) in the specimens of 32 PROM patients and 20 normal puerperas. The results were as follows: after HE stain, the infection rate in the PROM group was 46.88%(15/32), while in the normal group, it was 10.00% (2/20) ; Positive NF-κB, COX2 and MMP-9 immunoreactivity in fetal membranes in the PROM group was significantly higher than that in the normal group (P〈0. 05) ; The expression of NF-κB, COX-2 and MMP-9 in the chorioamnionitis group was higher than that in the nowchorioamnionitis group (P〈0.05) ; There was significantly positive correlation among NF-κB, COX-2 and MMP-9 (P〈0.01). It was concluded that the high expression of NF-κB , COX-2 and MMP-9 may play an important role in the pathogenesis of premature rupture of membrane. The expression of COX-2 and MMP-9 was regulated by NF-κB. The activation of NF-κB may be a key point in the etiological factor of PROM.
