三维快速平衡稳态进动MRI对三叉神经痛的诊断研究

摘　　要：目的探讨磁共振三维快速平衡稳态进动(3D-FIESTA)序列对三叉神经痛(trigeminal neuralgia,TN)患者神经血管关系的诊断价值及可靠性。方法对25例正常对照及25例TN患者三叉神经行3D-FIESTA序列成像并多平面重建,记录该序列对神经血管关系的诊断结果,其中15例与手术结果对照。结果正常对照组双侧及TN组患侧神经血管接触或压迫的阳性率分别为24%与87.5%,二者有非常显著性差异(P<0.001)。3D-FIESTA对15例TN患者诊断的敏感度、准确率分别为93%与67%。结论 3D-FIESTA序列是诊断神经血管关系较为敏感可靠的序列,可作为TN患者的首选序列。Objective To study the diagnostic value of 3-dimension fast imaging employing steady state acquisition（3D-FIESTA）in demonstrating neurovascular relationship in patients with trigeminal neuralgia（TN）.Methods 3D-FIESTA was performed in twenty-five volunteers and twenty-five patients with TN.The MRI findings of the sequence were recorded,operative results of 15 patients were compared with MR findings.Results Neurovascular contact could be identified in 87.5%（21/24） of the patients with TN,and 24%（12/50）of volunteers.The difference between the two groups was statistically significant（P〈0.001）.The sensitivity and accuracy of 3D-FIESTA was 93% and 67% respectivley.Conclusion 3D-FIESTA can clearly demonstrate the relationship of the nerve to surrounding blood vessels,it should be the primary MRI sequence for patients with TN.
