介绍一种处理胰内段胆总管囊肿的新手术方法(附12例效果分析)

摘　　要：目的介绍一种对胰内段胆总管囊肿处理的新方法 ,并分析其疗效。方法 12例患者接受该手术方式。胆总管囊肿切除至胰内段,距囊肿远端开口1 cm处行内荷包缝合关闭,石炭酸烧灼残留囊肿粘膜,缝合关闭残留囊肿腔。结果术中出血量为40-160 ml,平均（80.7±55.4）ml,手术时间为60-160 min,平均为（110.3±40.2）min,住院天数5-12 d,平均为（8.3±3.4）d,无并发症。均获随访,平均25.6（2-45）个月,无癌变及感染病例。结论该手术方法针对解剖困难、手术风险大的胰内段胆总管囊肿而言是有效、安全、简单的处理方式。Objective To study the effect of a modified surgical procedure for intrapancreatic section of choledochal cyst.Method 12 cases of choledochal cyst with difficult intrapancreatic dissection underwent the procedure,which described as that the cyst was removed as much as possible except the intrapancreatic section,distal part of cyst was closed by a purse string suture which was placed intraluminally about 1cm proximal to the distal end of cyst,carbolic acid was used to inactivate the residual cystic mucosa,and the residual lumen was closed by suturing.Results Blood loss was（80.7±55.4 ml）（40～160 ml）,operative time was（110.3±40.2 min）（60～160 min）,hospital stay was（8.3±3.4）days（5～12 days）,no complications was occurred.No malignancy or abscess was observed within 2 to 45 months of follow-up.Conclusion For those intrapancreatic section of choledochal cyst requiring highly risky dissection,the modified procedure is effective,safe and simple.
