超声引导瘤内注射凝血酶对兔VX2皮下移植瘤的影响

摘　　要：目的探讨超声引导下瘤内注射凝血酶对实体肿瘤生长的影响。方法 48只新西兰大白兔建立VX2皮下移植瘤模型,随机平均分为6组:对照组(0.9%生理盐水);超声引导下瘤内注射凝血酶50 U/ml、100 U/ml、150 U/ml、200 U/ml和250 U/ml组。在实验前1 d,实验第3、14天监测凝血功能变化情况,同时三维超声观测肿瘤生长情况及肿瘤血管生成变化;处死瘤兔后,肿瘤组织行用TUNEL法检测细胞凋亡指数。结果随着浓度增高,抑瘤率及凋亡指数均增加(P<0.05),200 U/ml与250 U/ml组比较差异无统计学意义(P>0.05),凝血功能中纤维蛋白原(FIB)先升高后降低,凝血酶原时间(PT)、活化的部分凝血活酶时间(APTT)无明显变化。结论超声引导下瘤内注射凝血酶可抑制肿瘤生长,且具有一定的浓度依赖性,凝血功能不发生显著改变。Objective To explore the effects of intratumoral injection of thrombin under ultrasound guidance on noumenal tumor grouth. Methods The model of subcutaneously transplanted VX2 tumor was estabtished in 48 rabbits, then they were divided into 6 groups randomly, which were treated with thrombin of 50 U/ml, 100 U/ml, 150 U/ml, 200 U/ml, 250 U/ml, and 0.9% NS （control group） , respectively. With ultrasound-guided puncture, thrombin was injected into the VX2 tumors in rabbits. The blood clotting function of rabbits was detected respectively on the 1 d pretherapy, the 3 d and 14 d posttreatment, the tumor growth and tumor vascularization in each group was measured by three-dimensional uhrasonography; then the tumor tissue was examined by pathology, and the apoptosis induction was detected with TUNEL assay. Results The tumor growth inhibition and the apoptotic index were improved with the does increased （ P 〈 0.05 ） , while there was no significant difference between 200 U/ml group and 250 U/ml group （ P 〉 0.05 ）. There was no significant difference in the blood clotting function presented by PT and APTT between pre and post treatment （ P 〈 0.05 ）, FIB was raised at the 3 d post-treatment and dropped back to normal level at the 14 d （ P 〉 0.05 ）. Conclusion Intratumoral injection of thromhin under ultrasound guidance can inhibit the growth of the subcutaneously transplanted VX2 tumor in a close-dependent manner, the treatment with thrombin does not cause marked change of the blood clotting.
